Angle Class II division 1 malocclusion is characterized bymaxillary protrusion, mandible retrusion, upper anterior tooth anteversion and anterior deep overjet and overbite. Different vertically malocclusions have their distinct face structural feature, and their response to orthodontic treatment are completely different. The difference of bioactivity and reactive site of soft and skeletal tissue are the main factors that influence the curative effects of adolescent and adult with Class II division 1 malocculsion with vertical growth pattern. Clinically, there is no thorough investigation on the change of soft and skeletal tissue and analysis of the relationships of malocclusion and orthodontic treatment.This study is aim to evaluate the treatment outcomes of patients of the affiliated hospital of West China Stomatology with Class II division 1 malocculsion with vertical growth pattern by orthodontic treatment of four first premolars extraction; to compare the differences of changes of soft and skeletal tissue between adolescent and adult,and to investigate the interaction between orthodontic treatments with growth and development; and to analyze the dependability of the improvement of profile esthetic index, to provide experimental results to help to make orthodontic treatment plan and treatments.38 patients (20 adolescent, 18 adult) from the Orthodontic Division of the affiliated hospital of West China Stomatology were included in our study. Their pre- and post-treatment cephalometric X-rays were measured. These patients were all consistent with the internalizing standards.The results show:1)Clinically, the orthodontic treatment results of the patients with Class II division 1 malocculsion with vertical growth pattern were treated effectivly by extraction four 1st premolars. The lower anterior teeth had not been tipped severely, the lower molar moved forward bodyly instead of tiltingly2)In the adolescent group, patients could express more skeletal effect, consequently,their profiles were more improved.3)Because of the different soft tissue remodelling between adolescent and adult, the ratio of adduction of upper anterior teeth and upper lip of adolescent was 4:1 or so, but that of adult was 2.8:1; the upper and lower lips of adolescent were longer and thicker after orthodontic treatment,but the lips rebuilding of adult were nearly no changes.4)Orthodontic treatment could provoke the inhibited developmental potency to release, and promote the development of mandible in adolescent group,even the mandible of adult had slight growth. This also suggests that the best treatment period of patients with Class II division 1 malocculsion with vertical growth pattern was in the rapid growth and development period of adolescent5)The factors influence improvement of esthetic indexs are the anteroposterior relationship of the maxilla and mandible,the location of chin and the anterior teeth in alveolar bone. These are more distinct in adolescent.
