Objective: To understand the rural children's guardians' Knowledge, Attitude andPractice(KAP) of the Expanded Program on Immunization (EPI), analyse theinfluential factors and explore the feasible ways which can improve the rurolchildren's immunization program and help to hold the immunization programpublicity and health education activities to the children's guardians.Methods: KAP surveys were conducted to the children's guardians with at leastone child aged≤12 years at Pingbian and Xiaxi townships in Pingshan county,Yibin city, Sichuan province. Hepatitis B serological examination for children wasconducted in the primary school. Comprehensive interventions were conducted justin Pingbian township community, and the interventional effect was evalued.Results: All of the guardians' EPI knowledge scores are low in the two townships,the average is 19.93, the passing rate is 0%. Children's age, nationality, educationlevel of parents are the main influencing factors to EPI knowledge scores. Parents'accesses to EPI knowledge and information are little, mainly from publicity ofdoctors and supporting staffs. Guardians' attitude to EPI is related to theirnationalities, education levels and EPI knowledge. The ratio of the children whohave never participated in the EPI was higher in Pingbian than that in Xiaxi. Themain affecting factors to whether the Children participated in the EPI are theirnationalities, the number of population and children each home; who is children'sGuardians, parents' education and guardians' KAP-score, whether guardian isinitial to take their children to receive the EPI. After reseiving health education onEPI at Pingbian township, the guardians'EPI knowledge average scores were increased by 1.51 times than the baseline scores; the proportion of Guardians whohad't heared of the EPI knowledge had reduced the 19.4%. The proportion ofgaining the EPI knowledge from distribution of publicity materials, relatives andfriends in Pingbian is higher than that in xiaxi as the same as being ready to receiveEPI knowledge through newspapers and magazines, friends and relatives' convey.In Pingbian the ratio that parents are willing activelly to bring their children to takethe vaccination is increased from 54.3%to 86.0%, which is also higher than that inxiaxi.Conclusions: At present, the situation on KAP of EPI is not optimistic, in Pinshancounty, Yibin City. Low education level of children's parents, little publicity onchild immunization, lack of the knowledge on EPI, concept and awareness are themain factors for the poor KAP on children EPI in remote rural areas. The EPIcommunity health education interventions conducted in Pingbian can makeguardians get more EPI-related knowledge. The guardians' go-aheadisrn ofVaccination has got certain amount of change, but the children's EPI situationhasn't evidently changed at present. Community health education interventionsachieved some effective results in the target sites, and developed a number ofhuman resources, who can ensure the sustainability of similar interventions projectin future.
