| Objective:To study the relationship among aortic blood pressure, brachial artery blood pressure and the severity of coronary artery stenosis. Further for the prevention and treatment of coronary heart disease.Methods:1.186 patiernts who underwent coronary angiography were divided into two groups: hypertension and normotension. According to the coronary angiography results, then divied the two groups into coronary heart disease and no coronary heart disease respectively .2.Aortic systolic bloodpressure(SBP) and diastolic blood pressure(DBP) were measured during the period of coronary angiography. brachial blood pressure were measured before and after coronary angiography.3.The coronary risk factors of age,sex,heart rate,diabetes,blood fat,uric acid and plasma fibrin were recorded and compared with groups.4.In hypertension group and normotension group the severity of coronary artery lesions was evaluated by Gensinis coronary artery score. Mean ,t-test ,and ANOVA analysis, multifactor correlation and regression were performed on SPSS12.0Result:1. In hypertension group,the average age, diabetes ratio,blood pressure and the ratio of coronary artery 3 vessels stenosis are increased significantly campared with normotension group(P<0.05).2. Both in the hypertension group and normotension group,the aortic blood pressure is higher than brachial artery blood pressure(P<0.05).And the result of paired T-test with aortic blood pressure and brachial artery blood pressure, demonstrate that aortic blood pressure is also higher than brachial artery blood pressure both in the hypertension group and normotension group.3. According to the coronary angiography results, then divied the hypertension group into coronary heart disease and no coronary heart disease two groups and compared.The results demonstrate that aortic systolic pressure and pulse pressure in the coronary heart disease group are higher than no coronary heart disease group,and have direct correlation with Gensinis coronary artery scores(P<0.05).The others risk factors are not statistically significant. And aortic systolic pressure and pulse pressure are independent risk factors of coronary heart disease accorded to the result of regression analysis.4. According to the coronary angiography results, then divied the normotension group into coronary heart disease and no coronary heart disease two groups and compared.The results demonstrate that aortic systolic pulse pressure in the coronary heart disease group are higher than no coronary heart disease group,and have direct correlation with Gensinis coronary artery scores. But the aortic pulse pressure is not independent risk factor of coronary heart disease when correct the effect of age in regression analysis.Conclusion:1. Hypertension is a disease affect by multiple factors. In hypertension group,the ratio of coronary heart disease and coronary artery 3 vessels stenosis are increased compare with the normotension group . .2. Brachial artery blood pressure is underestimate the aortic blood pressure,and should pay more attention in clinic practice. The aortic blood pressure can direct react the blood pressure of coronary artery,the value to predict coronary heart disease is better than brachial artery blood pressure.3. In order to prevent coronary heart disease effcetively, we should not only control the peripheral upper limb blood pressure but also the aortic systolic pulse pressure and pulse pressure in the patients with hypertension. |
