Expression of alpha smooth muscle actin is the indicate of hepatic stellate cell activating.And it has been extensive used in scientific research of liver ailment.Which has been proved that along whith zhe degree of hepatic fibrosis and other organ fibrosis,the expression of alpha smooth muscle actin increased.Arterial blood dominance and portal flow paucity were closely correlated with growth processes in the early stage of HCC.The exceptional artery also could be confirmed by the increasing expression of alpha smooth muscle actinAlpha smooth muscle actin antibody immunohistochemistry would provide a simple and biologically significant method which is usable to screen the degree of hepatic fibrosis,neovascularization and arterial blood supply in hepatocellular carcinoma, and may have predicting utility for patients outcome. This technique may be useful as an adjunct to surgical pathology of hepatocellular carcinoma.Object:To observe the information of the expression of alpha smooth muscle actin in hepatocellular carcinoma as well as the relationgship of its expression with the diameter of the tumor, with the expression in non-cancerous parts of the liver and with the expression when portal vein tumor thrombus are formed.Method:Postoperational specimens from 50 patients with hepatocellular carcinoma were immunostained for alpha smooth muscle actin. All of the specimens were divided into three groups according to the diamemter of the tumor that was less than three centimeters (9 cases), three to five centimeters(20 cases) and more than five centimeters(21 cases).Mensuration the IOD value of the expression of alpha smooth muscle actin in cancerous and non- cancerous tissues using the technique of immunohistochemistry by immunohistochemistry image analysis system and semiquantitative analysis, using the technique of immunohistochemistry.Result: Alpha smooth muscle actin positively expressed in all of the cancerous and non- cancerous tissues.The expression intensity in non-cancerous tissues was higher than that in cancerous tissues.With the tumor accretion,the expression value of alpha smooth muscle actin in cancerous and non-cancerous tissues raised up. (cancerous tissues: <3CM, 87.57±26.58; 3-5CM, 141.07±19.22; >5CM, 162.65±9.09; contrast in interblock:P<0.01. non-cancerous tissues: <3CM, 143.64±20.16; 3-5CM, 171.56±15.14; >5CM, 186.45±8.41; contrast in interblock:P<0.01 . ) Accompanying portal vein tumor thrombus ,the expression value ofα-SMA in cancerous and non-cancerous tissues was higher than that without portal vein tumor thrombus.Conclusion: With the tumor accretion,the expression value ofα-SMA in cancerous and non- cancerous tissues of hepatocellular carcinoma raises up; Accompanying portal vein tumor thrombus ,the expression value in cancerous and non- cancerous tissues is higher than that without portal vein tumor thrombus; Detecting the expression value may may have predicting utility for patients prognosis.
