| Objective:The purpose of this research is to analyze the impact of multiple influencing factors on neurological impairment to patients with acute cerebral arterial thrombosis.Methods:Prospective studies on the influencing factors to neurological impairment have been conducted on 135 patients with acute cerebral arterial thrombosis who satisfied the conditions for admission to our Department of Neurology during June 2006 to December 2006. We recorded patients'age, gender and blood pressure at time of admission, performed head CT and/or MRI, measured the size of infraction as well as position using Efilm workstation V2.1 and 3D-DOCTOR V3.5 image-scan software. All patients were required to fast in the next morning after admission and their blood samples were collected to evaluate biochemical indicators including hs-CRP(high sensitive-C reactive protein), HCY(homocysteine), FBS(fasting blood glucose), TC(cholesterol), TG(triglyceride) and FG(fibrinogen). To evaluate patients'degree of neurological impairment, their NDS and BI were evaluated and scored at time of admission as well as after one month's treatment. The collected data were analyzed through a multivariate gradual regressive approach using statistical software SPSS13.0.Results:Only FG, TC, DBP(diastolic blood pressure), and PP(pulse pressure) followed normal distribution (P > 0.05) while other indicators did not. All the data were simply corrected to each other. For examples, Hs-CRP had low- degree positive correlation with FBS, size of infraction and SBP, so had HCY with age, and Neurological Impairment Degree Score (NDS) 3 (NDS score 2– NDS score 1). However, HCY did not enter into the final regression model. The primary influencing factors to NDS score 1, in decreasing order of impact are size of infraction, position of infraction, hs-CRP and FBS. While the primary influencing factors to NDS score3 at the end of 1month treatment, in decreasing order of impact, are NDS score1, size of infraction, FG, hs-CRP and TC. Conclusions:Size of infraction and hs-CRP are very important determining factors in both NDS score 1 at the beginning of the treatment and NDS score 3 at the end of 1month treatment. However, in determining NDS score 3 at the end of 1month treatment, the impact of FG is greater than hs-CPR. The other influencing factors for NDS score1 at the beginning of the treatment include position of infraction(Lobe of brain)and FBS, while other influencing factors for NDS score3 also include NDS score 1, size of infraction and TC.So we can conclusion that it is helpful to recover from neurologic impairment through lowing these influencing factors directly by medication. |
