【Objective】The objective of this research is to detect the infected condition of human papillomavirus(HPV)gene subgroup in normal cervix,cervical intraepithelial neoplasias(CIN)and carcinoma of the cervix,analyze the infected relationship between normal cervix CIN cervical cancer and different genetypes of HPV, the dependability between HPV16/18 and cervica process, l approach the carcinogenicity of infection of different HPVgenetype,and provide theory evidence to prevent cervical cancer and explore gene therapy of cervical cancer.【Methods】Collected 564cases , which took cervical fluid basal cell examinations and deployed nucleonic acid molecule tacho-mixtus geneotyping technique HybriMax method to detect typings of 21kinds of HPV-DNA genetypes in cervical cells. The abnormal cases can take cervix multiple punch biopsy by colposcope cervical canals scoraping or LEEP and pathology examination of cervical tissue by total hysterectomy. According to the results of histopathology ,we could devided the cases into 3 groups which were cervicitis CIN and cervical cancer. Choose paraffin sections of another 47 cases from 564 cases to take HPV16/18 detection by hybridization in situ.【Results】1 The most frequent 10 genetypes in patients with cercival pathologic process who infect HPV (according to the decrement of positive rate)are: HPV16(18.5%),52 (15.5%),58(9.0%),68(8.8%),18(8.6%),cp8034(8.1%),33(7.6%),31(6.5%),11(5.6%),39(5.1%);inflammation group HPV52(14.7%),39(8.5%),68(7.8%),11(6.2%),16(5.4%),18(3.9%),56(3.1%),33(2.3%),66(2.3%),cp8034(2.3%);CINⅠgroup 16(12.1%),68(11.5%),52(10.9%),cp8034(9.2%),11(8.0%),18(8.0%),33(6.9%),58(6.9%),51(6.3%),56(5.7%);CIN II group16(30.3%),52(19.7%),58(16.7%),cp8034(12.1%),31(10.6%),33(10.6%),51(10.6%),18(7.6%),53(6.1%),68(6.1%);CIN III group16(45.8%),52(22.9%),58(20.8%),33(16.7%),cp8034(12.5%),18(12.5%),31(12.5%),51(10.6%),53(6.3%),68(6.3%);cercival cancer group16(45.8%),18(46.7%),52(33.3%),58(26.7%),33(20.0%),31(13.3%),53(13.3%),cp8034(13.3%),11(6.7%),68(6.7%).2 HPV infection has close relation with cervical process: Total positives of patient with cytology abnormal is 70.1%(303/432),positive rate of inflammatory group is 41.9% (54/129),positive rate of CINI,II and III are as follow 75.9%(132/174),86.4%(57/66) and 93.8%(45/48), positive rate of cervical cancer is 100.0%(15/15),however, positive rate of HPV infection is 26.5%(35/132) in patient with cytology normal.3 The condition of HPV16/18 infection: We didn't detect HPV16/18-DNA in normal cervical tissues which normal in hybridization in situ detection,the expression rate is 12.5%(1/8)in CINⅠ,Ⅱ, 30.0%(3/10)in CINⅢ, 55.6%(5/9)in squamous carcinoma of the cervix(SCC). There is much more significant distinction in HPV16/18 infection in SCC group comparing with normal group(P<0.001), where there is distinction in SCC comparing with CIN(P<0.05). Detect cervical cytotype with HybriMax method to know that HPV16/18 infection rate in SCC CINⅢand CINⅠ,Ⅱare as florrow66.7%(6/9),40.0%(4/10) and 12.5%(1/8), which shows that the infection rate in SCC is obviously higher than the other two groups, and significant distinction(P<0.001)comparing with the other two.4 There are multiple infections in cervical process with HPV infection, and the multiple infection rate reaches 42.6%(144/338) which can be increased accompany with cervical process's degree increasing.【Conclusions】1 It's frequent to infect 16,52,58,68,18,cp8034,33,31 and 11 types of HPV in patient with cervical process of this experiment, and HPV16,52,58,68,18 and cp8034 have more carcinogenicity than others.2 HPV infection rate can increase gradually accompanying with the severity of cervical process's degree, and cervical cancer group has significant deviation comparing with normal cervix group and CIN group.3 High risk HPV infection is an important factor to cervical cancer, SCC has obviously relation with HPV16/18 infection.4 Multiple infections of HPV maybe prompte carcinogenesis of cervix.5 HPV-DNA typing detection plays an important part in clinical diagnosis and predicting prognosis of patient with cytology abnormal.
