Objective: To study the expression of the Desmin albumen and Monocytechemoattractant protein-1 and(MCP-1) in renal of hypothyroidism rats,revealed the pathology of occurrence and development of the renal injury in hypothyroidism and the relationship between thyroid function and Mcp-1 and Desmin albumen and the intervention effects of Levothyroxine Sodium and stopping giving Propylthiouracil.And rectifying the thyroid function quickly and slowly had different influence on the renal injury resuming or not.Methods: The model of hypothyroidism rat was made by giving them propylthiouracil.Since the rats were given propylthiouracil for 8 weeks,the rats were divided into 3 groups which were giving propylthiouracil(B1 group),stopping giving propylthiouracil(B2 group) and giving Levothyroxine Sodium(B3 group),at the same time the rats lavaged by distilled water were named control group.The pathology condition of the control group,hypothyroidism group and intervention group(B2 group and B3 group)were observed in week 3,6,8,10,and 12.And using the immunohistochemistry to determine the expression of Mcp-1 and Desmin albumen in renal tissue. Results: 1 Symptom and physical sign: The rats whose condition in drinking and eating, mental state, daily activities in control group were normal. Give the propylthiouracil in week 3 the hypothyroidism rats gradually showed the symptoms of drinking and eating less than the control group, and they lost weight and reduced activity, and the reaction towards the surrounding was indifference. Some hypothyroidism rats had trichomadesis and rarefaction. The symptoms in B2 and B3 groups were better than the hypothyroidism group.2 the thyroid function examination:The level of FT3 and FT4 of hypothyroidism group was more significantly decreased than control group at week 3,6,8,10 and 12, which was aggravated progressively. The level of TSH was more significantly increased than control group. The thyroid function in B3 group was much better than the hypothyroidism group in week 10 and 12. The thyroid function in B2 group was much better than the hypothyroidism group in week 12. The thyroid function in B2 group and in B3 group had no more difference.3 Biochemical examination:Blood fat: The TC, TG, LDL of hypothyroidism group were increased than control group at week 3, 6, 8, 10, and 12, but HDL was increased than control group at week6, 8, 10and 12. The blood fat in B3 group was much better than the hypothyroidism group in week10 and 12. The blood fat in B2 group was much better than the hypothyroidism group in 12. The blood fat in B2 group and in B3 group had no more difference.Renal function: The level of of BUN and Crea of hypothyroidism group were more significantly increased than control group at week 6, 8, 10 and 12, which was aggravated progressively. The renal function in B3 group was much better than the hypothyroidism group in week 10 and 12. The renal function in B2 group was much better than the hypothyroidism group in week 12. The renal function in B2 group and in B3 group had difference only in week 12.4 Pathological changes: Light microscop in the control group there are not obviously abnormality in glomcrulus and renal interstitium. In the hypothyroidism group the connective tissue and phlegmonosis cells between the nephric tubule are more than the control group.They also has autolysis in proximal nephric tubule cell endochylema and anastole of glomerulus.Electron microscope: In the control group the structure of podocytes in glomcrulus is integrity, foot process lining up in order and clearly. In the hypothyroidism group glomerular basement membrane coiling irregularity; foot process of podocytes coalesce focally; the antilinear of capillary endothelium window increasing; crista mitochondriales of podocytes meromixis or to disappear; there is degranulation in rough endoplasmic reticulum; all of these aggravate following hypothyroidism condition.The pathological changes in B2 and B3 group were relieved more than the B1 group. 5 The Immunohistochemical staining: comparing to the control group, the expression of MCP-1 and Desmin were enhanced along with the course of disease in the hypothyroidism group. In B2 and B3 group those expression were better than the B1 group. The expression of those in B2 group and in B3 group had difference in week 10 and 12.6 Correlation: the increase of BUN,Crea were significantly positive correlation with TSH, and negative correlation with FT3, FT4. The increase of TC and TG were significantly negative correlation with FT3, FT4, positive correlation with TSH but LDL and HDL showed no correlation with FT3, FT4. The results of the MCP-1,Desmin were significantly negative correlation with FT3, FT4, and positive correlation with TSH.Conclusion: In the present study, the model of hypothyroidism rats was made by giving them propylthiouracil. Since the propylthiouracil was given for 3 weeks, the rats showed up Hypothyroidism symptoms and physical signs. And the thyroid function was consistent with Hypothyroidism. With the development of the disease, Hypothyroidism rats had showed kidney injury and the disturbance of lip metabolism, which displayed by the renal function, MCP-1 and blood fat's increase.The process coincidenced in path of physiological hypothyroidism process and the pathological changes of renal.The observation of the Desmin showed that the kidney injury was related with podocyte, which paralleled with the thyroid function. Treating with LT4 could ameliorate the thyroid function which could lessen the kidney injury and decrease the kidney function and blood fat, then delay the renal injury and protect the kidney.So did stopping giving PTU. But the effect of treating with LT4 was better than the one of stopping giving PTU. So rectifying the thyroid function quickly had better effect in the kidney injury than rectifying them slowly.
