Research Of Characteristic And Medical Care Cost Of Tuberrculosis Inpatients In A Changsha Hospital

Objective: By analyzing the tuberculosis inpatients medical care coststructure of a hospital in changsha in 2005,from the aspects of preventingand managing tuberculosis, we try to perfect the standard of medical caremanagement, strengthen the controlling of medical care quality, provideexact evidence for enhancing medical care benefit, and consummate thecorrelative policy.Methods: Date base of medical care cost about the tuberculosis inpatientswas been build by SPSS11.0. and it was been statistical described toage,days of hospital stay of tuberculosis inpatients and analysed the medicalcare cost's structure from the following three aspects: types of disease,payment mode and the result of cure. Ultimately we analysed the proportionof medication cost among the overall medical care cost and the medical carecost of average days of hospital stay.Results: Analyzing from age of tuberculosis inpatients, the retire inpatientsand throat tuberculosis inpatients are older than others. Analysing from daysof hospital stay for tuberculosis inpatients, hospital stay of the younginpatients, the retired inpatients and bone-tuberculosis inpatients arerelatively longer. In the aspect of medical care cost structure, analyzing fromage grouping, the overall medical care cost of the group of 1～9 years-oldand 10～19 years-old are relatively higher, that have the higher expenditureof bed, expenditure of diagnostic, expenditure of examination, theexpenditure of cure, expenditure of operation, expenditure of material, expenditure of anaesthesia and other medical expenditure than the othergroup of age. Analysing from payment mode, the overall medical care costof the retired medicare inpatients are the highest, whose expenditure ofmedication, expenditure of bed, expenditure of examination,the expenditureof cure, expenditure of nurse,the expenditure of assay, expenditure ofmaterial and other medical expenditure are relative higher. Analysing fromthe type of tuberculosis, the overall medical care cost of tuberculo-meningitis, bone-tuberculosis and bronchia-tuberculosis are relative higher.And the expenditure of examination, expenditure of examination,expenditure of nurse, expenditure of material of tuberculo- meningitis.expenditure of operation of bronchia-tuberculosis are the highest. Analysingfrom the cure result of tuberculosis, the overall medical care cost,expenditure of medication, expenditure of operation, the expenditure ofassay, expenditure of anaesthesia of the group of unhealing are the highest.In addition, the expenditure of diagnostic, expenditure of nurse, expenditureof material and other medical expenditure of the death group are higher thanother groups. Average days of hospital stay of the tuberculosis inpatients is32.47 days. Average medical care cost is 8213.63 yuan, the days of hospitalstay shows downtrend. Each bed medical care cost of each day showsascendtrend. The average medical cost of tuberculosis inpatients is 3966.71yuan, the proportion of which among the overall medical care cost isincreased.Conclusion: All medical care cost of the tuberculosis inpatients willincrease, if their days of hospital stay is long and the illness is sever. Whenthe threapy plan of illness is different, the structure of medical care cost isdifferent as well. In addition, the structure of medical care cost of differentillness is different. Therefore, by strengthening the medical care quality control, perfecting medical care process, shortening the average days ofhospital stay, strictly controlling the medical care cost in a long time, wecould promote the hospital development in healthy way.
