Objective: To define the relation between sequential myocardial injury andTNF-∝in critical ill patients.Methods: Forty-eight cases were evaluated with APCHEⅡ,The serum wasobtained in 1d,3d,7d,and cardiac troponinⅠ(cTnI) and tumor necrosisfactor-∝(TNF-∝), interleukin-6 (IL6) were determined. Forty patientswere divided into contrast group(n=20) and ulinastatin-treated group(n=20,UTI group). APCHEⅡof the two group are unsignificantly. The patients inUTI group were given 20mg ulinastatin intravenously immediately afteradmission, 3 times a day for a week. The serum was obtained in 3d,7d,andcTnI and TNF-∝, IL6 were determined.Results: Morbidity of sequential myocardial injury was 23%. And the serumlevels of cTnI are correlated with TNF-∝and IL6. The plasma content ofcTnI, and TNF-∝, IL6 of the UTI group were significantly lowerer than thecontrast group (P<0.05),Conclusions: The morbidity shows a relationship between sequentialmyocardial injury and cytokines.
