Objective: To retrospective evaluate the accuracy of endoscopic ultrasonograpby (EUS) in detemination of pancreatic cancer: analyze the value of endoscopic ultrasonography (EUS) in detecting the TNM staging; investigate the clinical utiity and safety of endoscopic ultrasonography-guided fine needle aspiration biopsy (EUS-FNAB) for diagnosis of pancreatic cancer。Methods: 15 patients (13 men and 2 women) with pancreatic leasions detected by CT scanining, ultrasonograpby or clinically suspectved diagnosis were referred toEUS。the size, shape, position, blood vessel and lymph node metastasis of the pancreatic lesion were assessed and stageed preoperatively by using the TNM system. Simultataneously, EUS-guided FNAB was carried out。The preoperative endosonographic results were compared with the histopathological Staging.Results: The lesions detected by CT scanning (13cases) and ultrasonography (12cases) were all confirmed by EUS (15 cases)。15 patient were performed EUS-guided FNAB and only 1 pantient faliled。Among the other 14 patient, cytology was determined in 12 and histology in 8。Cytology and pathology demonstrated pancreatic cancer or highly suspicious pancreatic cancer in 7 patients。The diagnostic sensitivity of EUS-FNAB was 80%, specificity100%。For TNM stage, the overall accordance rate of EUS in T stage and N staging were 62.5% and 50%。The results were confirmed by surgery in 8 patients。 Conclusion: Eus was a effective modality for diagnosis of pancreatic lesions and more valuable in the evaluation of the TNM stage of pancreatic carcinoma。The EUS-FNAB was a safe, reliable and accurate method for pancreatic cancer and can be widely used in clinical practic。... |
