Study Of Clinical Characteristics And Treatment Of Chronic Severe Hepatitis B With Intestinal Endotoximia By TCM

1 Objective1.1To observe the TCM symptom features of chronic severe hepatitis B with Intestinal Endotoximia, and study pathogenesis symptoms . To observe the curative effects of compund formula Qingchangyangganfang. To search a effective way to treat chronic severe hepatitis B with Intestinal Endotoximia.2 Methods2.1Case selectionAll the cases are hospitalized patients in Beijing Youan Hospital,from July,2006 to January,2007;aged from 17to 67. All the patients meet the diagnosis: chronic severe hepatitis B with Intestinal Endotoximia combined with intestinal endotoximia.2.2Sample collection and examine meaure of endotoxinAll patients were test endotoxin,some patient with ascites were gathered ascites to assay ascites endotoxin, the others were tested plasma endotoxin. The ascites specimen were operated by doctors in strict according to criterion of diagnosis and treatment . 5 ml ascites were gathered and assaied immediately by 10 ml sterile injector.2.3Therapetic regimen35 cases were divided into 2 groups according to the admission time,one for therapy group,another for contrast group. The basic treatment was similar to the two groups for example: antibiotics, energy sustain, dehydration, diuretic,albumin,etc.Qingchangyanggafang decoct was used in the therapy group by clyster for 2 weeks ,200ml ,5-7 times a week ,30minutes every time . After two weeks we observed the curative effects.2 .4Observations2 .4.1Observe the TCM symptom and signs of patients,score the main symptoms and signs, Compared the changes of total scores for the main symptoms between the therapy group and the contrast group.2 .4.2 Compare the changes of the liver function,renal function, ammonia and endotoxin quantitation of the two groups.3 ResultsTiredness,inappetence,severe abdominal distention,thirsty ,prompt and micro pulse ,dark red tongue,thick and greasy tongue fur,dry stool were observed in the patients of chronic severe hepatitis B with Intestinal Endotoximia intestinal endotoximia . Therapy group was better than contrast group on improving symptoms, eliminating endotoxin and lowering blood ammonia.4ConclusionThe patients in chronic severe hepatitis B with Intestinal Endotoximia included main symptoms for example tiredness,inappetence,dry stool,severe abdominal distention,thirsty ,prompt and micro pulse ,dark red tongue,thick and greasy tongue fur.The main pathogenesis was"toxin","stagnation","deficiency of hepatic".Qingchangyangganfang had the effects of nourishing hepatic essence depriving the evil stagnation,promoting blood flow,detoxicating,opening bowels.It showel better effects on improving main symptoms clearing endotoxin in this study。No side effects were observed.
