Objective: Hypertensive cerebral hemorrhage is common and frequentlyoccurring in cerebral disease. The death rate and remnant rate are high. It brings bigperplex and burden to society, family and personal. Formerly, people thinkhemorrhage is single phase process and quickly stop because of blood solidify andoppress from surrounding brain organization.With the development of image technique, specially extensive application of CT,people find that earlier haematoma extensions of hypertensive cerebral hemorrhageare not few, reaching to 35.5%-52%. It is one of the important reasons foraggravation of earlier patient's condition and earlier death, seriously influencing theprognosis. Therefore, it is very necessity that people study the characteristics andrelated factor on earlier haematoma extension of hypertensive cerebralhemorrhage.Method: The investigation is that 42 cases with earlier haematoma extensionof hypertensive cerebral hemorrhage were analyzed retrospectively. They are treatedbetween January, 2001 and January 2007 in the department of the nervous internalmedicine of the Dunhua city hospital of Jilin province. The clinical data are includingpreviou history, personal history, clinical performance, cerebral CT, laboratoryexamination, treatment measure and disease prognosis etc.Result: This study indicates that the average everyday capacity for liquor annualof 26 cases (61.90%) with long-term drinking is about 150 grams alcoholic for 10-20years in 42 cases with earlier haematoma extension of hypertensive cerebralhemorrhage. The first CT is checked within 24 hours. The all haematoma extensionsoccurred within 36 hours, but 26 cases (61.90%) occurred within 6 hours. Themajority of hemorrhage is in basal nuclei zone and thalamus, 31 cases (73.8%) in basal nuclei zone and thalamus.According to the first CT,the haematoma is divided into theregular shape (similar circular or kidney form) and irregular shape (30 cases,71.43%). The 34 cases are applied mannitol in the earliest period (the disease comingon within 6 hours).Conclusion: To sum up, the short-time occurrence, the hemorrhage in basal nucleizone and thalamus, the irregular shapes of haematomas,the remarkable hypertension,the application of mannitol in the earliest period and the long-term of drinking are theimportant factors of the earlier extension of haematomas.
