Objective To observe the urine sIgA concentration of differentgroups in orderto further explore the protective role of local immunity onpatients with upper urinary tract infections.Methods Urine sIgA content was measured by means of enzyme linkedimmunosorbent assay (ELISA) in the following groups:15 cases ofnormal, 14cases of acute cystitis, 19 cases of acute pyelonephritis and 12cases of Orthotopic continent delemial sigmoid neobladder operationrespectively. Its effects on preventing upper urinary tract inrectionsafter operation were also studied and estimated.Results An average sIgA concentration of urine (mg/g Cr) wasmeasured, which was 2.13 in normal group, 1.28 in acute cystitis, 27.67 inacute pyelonephritis and 522.26(P<0.001) in detenia ceacal-colon continent reservoir, 2.63 in preoperative and 514.76(P<0.001) inpostoperative cases respectively. It was also observed that theconcentration of urine sIgA in patients with upper urinary tract infectionsafter urine diversion was 481.29(mg/g Cr) while that of without infectionswas 551.61(P<0.45) and the bacteria cultivated from reservoir flow mainlywas Eschrichia coli bacillus and Kelebsiellus bacilles.Conclusion Urine sIgA content in cases of acute cystitis had nosignificant difference from that in normal people, which indicated that lowerlocal immuno-activity existed in bladder. As urine sIgA content in cases ofacute pyelonephritis was significantly higher than in normal people and thatin cystitis, which suggested that renal local immuno-reaction was much moreactive. Urine sIgA measurement might be used in tracing the treatment of renalinfection and was conductive to diagnosis of recessive pyelonephritis. Therewas no significant difference in sIgA content between infectd postoperativegroup and that without infection while the content of sIgA was ten timeshigher than that in normal group and lower urinary tract infection group.All these suggested that urine sIgA presented nonspecific immune effects.Although bacteria could be cultivated from reservoir, they didn't causeinfection definitely. Bacteria were essential factors for postoperativeinfection after detenia ceacal-colon continent reservoir, but it needed some other factors to cooperate.
