It mainly involved colonictunica mucosa and tela submucosa. Irritable bowel syndrome(IBS) was one of themost common functional gastrointestinal disorders(FGID). It can be subdivided intothree types: constipation-predominant IBS(C-IBS),diarrhea-predominantIBS(D-IBS) and alternating IBS. The incidence of the two colonic diseases hasincreased recently. Although the true pathogenesis of the two colonic diseases wasstill unknown, there were evidences that many ingredients including environment,inheritance, infection, allergy, psychological factors, and some immunologicalfactors may be involved in its development. Humanβ-defensin-2(HBD-2) was a kindof antibacterial peptides, and had antimicrobial activity. Nuclear factor kappaB(NF-κB) was a kind of protein factor which had the effect of transcription andaccommodation. Interleukin-8(IL-8)and tumor necrosis factor alfa(TNF-α) wereproinflammatory cytokines. The four factors were related with UC. It was not clearthe relationship between the four factors and D-IBS. We aimed to investigate theexpression of HBD-2,NF-κB,IL-8 and TNF-αin colonic mucosa in UC and D-IBS,and study the relationship and the clinic significance between the four parametersand the two diseases.Methods:Thirty patients with UC were collected and were divided into three groupsaccording to the clinical symptoms: mild, moderate and severe groups. Thebiopsy specimens were taken from areas of ulcerated colonic mucosa. Specimensfrom colonic mucosa in twenty D-IBS patients were collected. Meanwhile, specimens from colonic mucosa in ten normal persons were also collectedas controls. RT-PCR was used to examine the expression of HBD-2 at thelevel of mRNA. And immunohistochemistry was used to examine the expression ofHBD-2,NF-κB,IL-8 and TNF-α. The results of RT-PCR and immunohistochemistry were beanalyzed by One way ANOVA, Chi-square and Kruskal-Wallis H respectively. Andthe correlation between the four factors: HBD-2,NF-κB,IL-8 and TNF-α, wasanalyzed by spearman rank correlation.Results:1. On the mRNA level, the expression of HBD-2mRNA in the colonic mucosa ofUC,D-IBS and the controls existed. The expression of the HBD-2 in the colonicmucosa of UC was significantly higher than that in D-IBS and the normal colonicmucosa (P<0.01). But no significant difference was found between D-IBS and thenormal colonic mucosa (P>0.05). The expression of HBD-2mRNA had no statisticaldifference in colonic mucosa of mild,moderate and serve UC (P>0.05).2. It was shown by immunohistochemistry that the positive rate of HBD-2,NF-κB,IL-8 and TNF-αin the colonic mucosa of UC was significantly higher thanthat in D-IBS and the normal colonic mucosa(P<0.01). But no significant differencewas found between D-IBS and the normal colonic mucosa (P>0.05).The intensity of the expression of HBD-2,NF-κB,IL-8 and TNF-αin the colonicmucosa of UC was significantly higher than that in D-IBS and the normal colonicmucosa(P<0.01). But no significant difference was found between D-IBS and thenormal colonic mucosa (P>0.05).The intensity of expression of HBD-2,NF-κB,IL-8 and TNF-αin the colonicmucosa of mild,moderate and serve UC was higher than that in D-IBS and thenormal colonic mucosa(P<0.05).The positive rate and the intensity of the expression of HBD-2,NF-κB,IL-8 andTNF-αhad no statistical difference in colonic mucosa of mild,moderate and serve UC(P>0.05).3. There were significant correlations with the expression of HBD-2,NF-κB,IL-8 and TNF-α.Conclusion:1. The expression of HBD-2,NF-κB,IL-8 and TNF-αin the colonic mucosa ofUC was significantly higher than that in D-IBS and the normal colonic mucosa. Andthere were significant correlations with the expression of the four factors. It wasshown that the four indexes might play an important role in the development of UC.2. The expression of HBD-2,NF-κB,IL-8 and TNF-αhad no statistical differencein colonic mucosa of D-IBS and the controls. It was shown that the four indexes hadno correlation with D-IBS.3. The expression of HBD-2,NF-κB,IL-8 and TNF-αhad no statistical differencein colonic mucosa of mild,moderate and serve UC.
