The Staged Therapy Of Severe Pilon Fracture

Objective: To summarize the experences and lessons on the staged therapy of severe Pilon fracture and to evaluate its chinical effects.Methods From June of 2003 to January of 2006,11 cases of severe Pilon fracture were treated with open reduction and internal fixation in two stages therapy in our department and all of them were stabilized temporarily by calcaneal traction after injury with an average of 12.6 days (7 to 21 days). The open reduction and internal fixation were performed as soon as the soft tissue recovered. There were7 males and4 females with an average age of 36.2 years ( 18 to 59 years) .Among 11 cases,3 wereopened fractures and 8 were closed fractures.According to Ruedi-Allgowerclassification,the fractures were classified as type II in2 and typelI1 in 9.All of them were followed up.Result The final outcomes of treatment were evaluated based on ankle joint symptoms and functions score results and radiographic results. After 11 months (6 to 18 months) followed up,according to the scale of Mazur, there were 6 excellent cases,3 good cases,1 fair case,1 poor case. The good and excellent rate was 81.8%.Conclusion. The staged therapy of severe Pilon fracture which pays attention to the condition of soft tissue may avoid the complications of soft tissue. Meanwhile, in connection with other complications, it also has concrete treatment measure. In a word, it is a good method in the treatment of severe Pilon fracture.
