AIM: To investigate if expressive intensity of heat shock protein 70 would changed following progress of stress ulcer as well as investigate the relationship between the expression of HSP70 and prognosis。METHODS: Thirty patients(APACHEⅢ>18) of stress ulcer (divided into 1,2,3 step by state of an illness) from ICU and thirty patients of no-bleeding of pre-operation of selective operation from outside ICU were included as study subjects. Gastric fluid and blood specimen were collected from these subjects. To extract total-RNA from gastric juice cells and peripheral white blood cell (PWBC) through TRIzol-methods and then to investigate expression intensity of heat shock protein 70 by semi-quantitative RT-PCR and evaluate the HSP70 expression in PWBC by immunohistochemistry while record two-month survival rate. RESULTS:①Expressive intensity of heat shock protein 70 in each stages of stress ulcer was remarkably higher than that in the control group (blood specimen 0.5160±0.2737, Gastric fluid 0.5890±0.3211) (P<0.05); the expression of HSP70 of 2's stage (blood specimen 1.0490±0.2807, Gastric fluid 1.1079±0.4397) is higher than that of l's stage (blood specimen 0.7019±0.2347, Gastric fluid 0.8617±0.3634) and 3's stage (blood specimen 0.6801±0.2559, Gastric fluid 0.8507±0.3688) among patients of stress ulcer (P<0.05)but there are no significant differences in the expressive intensity of heat shock protein 70 between the l's stage and 3's stage (P>0.05);②Expression of HSP70 of survival team higher than fatality team, there was significant difference within 2's stage (F=7.878, P<0.01) and 3's stage (F=4.260, P<0.05) respectively, but there was not difference within l's stage (F= 2.874, P>0.05);③There was not difference between higher risk team and low risk team within experiment teams (within 1' stage F=0.155, P>0.05; within 2' stage F=0.025, P>0.05; within 3' stage F=0.243, P>0.05) . CONCLUSION:①Expression of heat shock protein 70 can be detected in gastric juice cell and PWBC of patients under stress after acute trauma and the highest expression of HSP70 was the most serious stage;②Expression of heat shock protein 70 of gastric juice cell and PWBC of survival team was higher markedly than fatality team; It's indicated that the higher expression of HSP70, the more chance patients to survival.
