Since there is close relationship between lung and blood vessels, the mechanism of the "lung faces the hundred vessels" can be further explained through deep study on that relationship. This task selected patients with qi deficiency syndrome of such chronic pulmonary diseases as COPD and late lung cancer to be studied. The NO, vWF(Von willebrand factor), t-PA, PAI-1 and t-PA/ PAI-1 were chosen to be the examine indexes. By observing the impact of qi deficiency syndrome of COPD and late lung cancer on the vascular endothelial cell function, the relationship between lung and blood vessels was explored and discussed, and thus, to provide the closest modern research indexes for the explanation of the TCM theory of "lung faces the hundred vessels" and the objective indexes evaluating the treatment effect of the chronic lung disease.I Literature studies on qi deficiency syndrome of COPD and late lung cancerThis study reviews the recognitions in ancient TCM literatures about the theory of "lung faces the hundred vessels", diseases such as cough, asthma, lung distension and lung accumulation, and the relationship between those diseases and lung qi deficency syndrome. It' s considered as follows: the lung governs qi of the whole body while heart governs blood of that. The blood flow depends on the promotion of qi. Zong qi goes up and out of the throat to help lung governing respiratory, while goes down through the heart vessels to promote the blood flow of the hundred vessels of the whole body, and thus maintains the flow infusing to tissues. COPD can be diagnosed as cough, asthma and lung distension etc. in TCM. Late lung cancer belongs to lung accumulation in TCM. Due to the long duration, lung qi deficiency syndrome often be seen in the chronic lung diseases like COPD and late lung cancer. It' s reflected that just a few domestic literatures have been focused on researches about the theory of "lung faces the hundred vessels", especially on the modern experimental researches. The modern experimental researches mainly include two aspects as follows: 1. Using the blood coagulation and anticoagulation substances in lung and the rheology to emphasize the adjustment of lung to the blood flow. 2. Using the vasoactive sustance to emphasize the adjustment of lung to vessels. "lung faces the hundred vessels", thus lung has the function of adjusting the vessels, which is related to the lung metabolic function.ⅡClinical experimental research on the impact of qi deficiency syndrome of COPD and late lung cancer on the vascular endothelial cell function1. Method: This task selected patients with qi deficiency syndrome of chronic pulmonary diseases such as COPD and late lung cancer to be studied. The changes of such indexes as NO, vWF(Von willebrand factor), t-PA, PAI-1 and t-PA/ PAI-lwere examined, the impact of qi deficiency syndrome of COPD and late lung cancer on the vascular endothelial cell function was discussed, and different diseases with the same syndrome were compared to one another to clarify whether they have different impacts on the vascular endothelial cell function. Therefore, the adjustment of lung to the vessels were clarified in terms of constraction and relaxation of microcirculation vessels, adhesion and gathering of thrombocyte and activity of blood coagulation and fibrinolytic system.2. Result:(1)To the patients with qi deficiency syndrome of such chronic pulmonary diseases as COPD and late lung cancer, before the treatment, the t-PA activity was decreased while the activity of PAI-1 and the amount of NO and vWF were increased, both were meaningful in the statistics(P<0.05); after the treatment, all of the indexes had been improved but had not to the normal standard. The result indicated that in the patients with qi deficiency syndrome, of chronic pulmonary diseases, such impairments of the vascular endothelial cell function as decreased constraction and relaxation of microcirculation vessels, higher degree of adhesion and gathering of thrombocyte and decreased activity of blood coagulation and fibrinolytic system were existed. After the combined therapies of Traditional Chinese medicine and Western medicine, the vascular endothelial cell function had been improved and the patients felt better. (2)Comparing to the normal group and before treatment, the t-PA in the two patient groups with qi deficiency syndrome of COPD and late lung cancer were both decreased(both P<0.05) whlie the lung cancer group gained a more significant reduction(P<0.001); the PAI-1 in the two patient groups were both obviously increased, and the difference of the COPD group reached significant(P<0.05) while the late lung cancer one didn' t(P>0.05); the vWF in the two patient groups were both increased compared with the normal group and the differences reached significant, but the lung cancer group gained a more significant rise; the NO in the two patient groups were both increased whlie the lung cancer group gained a more significant rise, but neither of the differences reached significant(P>0.05). The result indicated that, to the patients with qi deficiency syndrome of COPD and late lung cancer, the vascular endothelial cell function was impaired, the constraction and relaxation of microcirculation vessels were decreased, the thrombocytes were in higher degree of adhesion and gathering, and the activity of blood coagulation and fibrinolytic system were decreased. The lung cancer patients suffered more severe impairment, but the difference between them had not reached significant, and it may be because that the cases of lung cancer included were not adequate.(3) Conclusion:To the patients with qi deficiency syndrome of COPD and late lung cancer, the vascular endothelial cell function was decreased, the blood was in hypercoagulativity, both of which were in accordance with the TCM theory of "qi failing to promote blood". Being similar to the TCM theory of "lung faces the hundred vessels", the result may contribute to enrich the content of TCM fundamental theory study and to deepen the study of TCM theories. The indexes of this study were possible to be the cloest ones of modern experimental studies to explain the "lung faces the hundred vessels" and the objective index evaluating the clinical treatment effect of the chronic lung diseases. |
