The Curative Effect Of L-arginine On The Apoptosis Of Renal Cells Induced By Pancreatitis Associated Ascitic Fluid.

Objective: To explore the mechanism of acute renal injury in severe acutepancreatitis(SAP) and the conservative mechanism of L-arginine on it throughobserving the effect of pancreatitis associated ascitic fluid(PAAF) on the apoptosis ofnormal renal cells and the curative effect of L-arginine on the injury. Method: Theforty rats were established into SAP models by retrograde injection of 5ï¼…sodiumtaurocholate into the pancreatic duct and then collect the PAAF for reservation.Another forty rats were established into perforative peritonitis(PP) models and thencollect the perforative peritonitis associated ascitic fluid(PPAF) for reservation. Thelast forty SD rats were randomly divided into five groups: NS group,PAAF group,PPAF group, PAAF+L-arg group and PPAF+L-arg group. Serum concentrations ofamylase,urea nitrogen,creatinine and nitric oxide were determined at 12th hourpost-operation. The renal cell apoptosis rate and inducible nitric oxide synthaseactivity were examined. Histological observation was performed. Results: There wereno statistical differences between SAP group and PP group, PAAF group and PPAFgroup,PAAF+L-arg group and PPAF+L-arg group. The amylase,urea nitrogen,creatinine levels of serum in the PAAF group were higher than PAAF+L-arg group,while lower than SAP group. The amylase,urea nitrogen,creatinine levels of serum inthe PPAF group were higher than PPAF+L-arg group, while lower than PP group, andNS group was the lowest, which were identical with pathological changes and theapoptosis rate of renal cells. The content of nitric oxide and the activity of induciblenitric oxide synthase in the SAP group were higher than PAAF group, while lowerthan PAAF+L-Arg group, and NS group was the lowest. As the former like, PP groupwere higher than PPAF group, while lower than PPAF+L-arg group, and NS groupwas the lowest. Conclusion: The renal cell apoptosis and injury could be induced by pancreatitis associated ascitic fluid and it could be blocked by low doses of L-arginine.The stable renal injury model could be established by PAAF.
