Objective To explore/study the expression of the total serum IgE (T-IgE) in the AECOPD (Acute Exacerbation Chronic Obstructive Pulmonary Disease) patients and to observe if it can be a sign to guide glucocorticoids (GC) inhaled on them. Methods According to the Radom Digital Expressive Method, all the 90 cases with AECOPD were divided into the following two groups: the Glucocorticosteroid group (the GC group) and the control group, each group include 45 cases.The control group gave antibiotic and oxygen therapy and other routine treatment, the GC group gave fluticasone 250 ug inhaled twice daily besides the routine treatment。the level of total serum IgE were examined by EOSA both in the serum of the GC group and control group before treatment; Meanwhile , FEV1,FEV1%,FEV1/FVC,arterial blood gas analysis and was examined before and after treatment. Results 1,There was no significant difference on the lung function and the arterial blood gas analysis between the two groups before treatment (all P > 0.05 ); 2,After 7 days treatment the lung function and the arterial blood gas analysis got notable improvement not only in GC group but also in control group, in GC group all the index P < 0.01, in the control group the FEV1,FEV1/ FVC%,FEV1%P is 0.011 (t=2.65),0.015(t=2.53),0.05 (t=2.93) ;PH,PO2,PaCO2 P is 0.071 (t=1.95),0.022 (t=2.38,0.003 (t=3.19)。3,After treated the difference of all variable in GC group is more significant compare with the control group except the PH ( FEV1,FEV1%,FEV1/ FVC%,PO2,Pa CO2 P < 0.01) ; 4,According to the level of the total serum IgE before treatment, T-IgE > 100Iu/ml as a criterion , all patients was divided into two parts: the T-IgE Positive patients and the T-IgE Negative patients; After the GC inhaled treatment the T-IgE Positive patients got significant lung function change ( all P < 0.01) , when the T-IgE Negative patients got some change but did not get notable statistical change (all P > 0.05)。Conclusion GC inhaled is effective to AECOPD patients; 38% AECOPD patients get T-IgE Positive, T-IgE Positive patients is more effective to GC inhaled, it suggest theallergies I is part of pathology of AECOPD patients。Furthermore It suggests that the effects of GC on AECOPD partly come from the improvement of the allergic condition in trachea, then relieve the airway spasm。...
