Objective: By means of D- galactosamine inducing to acute hepatic failure animal model, and treat them with NAC to observe the effect of NAC and research its mechanism of action deeply.Method: Choose Chinese experimental small pig, female sex, twelve, to divide them into two groups at random. Control group (n=6)and treat group (n=6). By means of D- galactosamine 1.2g/Kg intravenous injection and inducing, we establish acute hepatic failure animal model. Treat group administrated six hours later intravenous dripped with NAC150mg/kg plugged into 100ml 5% glucose liquid. Twenty four hours later intravenous dripped with NAC 50mg/kg plugged into 100ml 5% glucose liquid. Forty eight hours later intravenous dripped with NAC 100mg/kg plugged into 100ml 5% glucose liquid, at last intravenous dripped with NAC100mg/kg plugged into 100ml 5% glucose liquid every 24 hours. Control group administrated six hours later, twenty four hours later intravenous dripped with 100ml 5% glucose liquid, and at last intravenous dripped with 100ml 5% glucose liquid every 24 hours. Observe and compare with common condition, live time, physiological and biochemical indicator, intracranial pressure, constitution pathological change and serology correlation factor change of two groups.Outcome: Control group mean survival time is 64.7±8.7hours, they all died of serious hepatic failure. Treat group mean survival time is 66.2±10.6hours. Comparing of mean survival time, the difference between two groups is no significant meaning (P = 0.73, P>0.05) .Comparing with control group, many biochemical indicators, intracranial pressure and inflammatory cell factors have disparity changed (p<0.05~p<0.01) in treat group.In pathology check,treat group constitution pathological change have fine improvement comparing with control group besides the sixty hours group. In immunity histochemistry check, PCNA of control group exist masculine expression, but the number is little, coloration is tint. The number of masculine expression is many and coloration is deep in treat group. By counting and statistical analysis masculine cell of two groups, the difference of masculine expression rate have significant meaning (p<0.05) . There are Caspase-3 masculine expression in both control group and treat group. Cell-substance and cell nucleus are all buffy grain, and the density is disparation. But the number of masculine expression grain in treat group is less than control group.Conclusion: NAC is effective to treat acute hepatic failure.①NAC can improve general condition and function of organism, and provide beneficial help to recover hepatic function at some extent.②NAC can decrease the production of physical TNF-a,IL-1βinflammatory cell factor, restrain fast development of Inflammatory reaction, stabilize physical internal environment, relieve hepatic damage, and supply condition for hepatocellular regeneration and recovery. NAC have some effect to sustain the balance of oxidation and antioxygen, increase the content of antioxygen SOD,reducibility glutathione and NO to clean up hepatic oxyradical production, relieve hepatic damage of oxyradical to protect liver.④NAC can promote hepatocellular regeneration to recover hepatic function. In a word, NAC provide an effective method and new choice for hepatic failure, and have significant clinical meaning. |
