Purpose To study the protective effects and possible mechanism of Ginkgo Biloba extract against renal damage induced by high energy ESW in rabbits. Methods 24 rabbits were randomly divided into 3 groups: sham therapy (physiological saline) as groupl, Inosine(control) as group2 and extract of Ginkgo Biloba as group3. these groups were intravenously given physiological saline(20ml/d). Inosine(80mg/kg.d) and extract of Ginkgo Biloba (30 mg/kg.d), respectively. 3 days before ESW was applied(1500shocks, 18kv, huikang-VI, made in shengzheng).then detected the expression of heme oxygenase-1 in renal tissue by immunohistochemistry and the level variation of urine N-acetyl-β-D-glucosaminidase(NAG), serum creatine (Cr), malonaldehyde (MDA), superoxide dismutase (SOD) and bilirubin in renal homogenate were observed. Results The heme oxygenase-1 protein expression can be significantly induced in renal tissue by EGb and intensived HO-1 activity;, the increase of index related to renal damage (MDA,NAG,Cr) in group2 and group3 was significantly lower than in groupl after shocking (P<0.05). And the values of them recovered to pre-shocking levels in 1 week respectively. MDA in group3 was not significantly elevated compared with pre-shocking(P>0.05) and the index related to renal protective SOD was significantly higher than groupl (P<0.05); furthermore, renal damage in group2 was milder than in groupl, but group3(EGb) was even milder and its recovery even sooner. Conclusions EGb can significantly improve the renal damage induced by ESW. its mechanism may relation to the expression of heme oxygenase-1 in renal tissue induced by EGb and intensive the capability of anti-oxygen and clean free radical.
