Objective: to observe the influence of using insulin in the treatment of the chronic traumatic surface, which is very difficult to concrescence, dicuss the possible mechanism, to seek the more correct, more easy and more economical method of curing the chronic traumatic surface, and to offer the standard of our clinical nursing.Method: subjects: according to the severity of the traumatic surface, divide the subjects into three groups: gentle, interim and severe, the subjects whose trauma has the same level are divided into I and II groups, group I is control group and group II is the experimental group(except using insulin). Observe and record the cure time and efficiency after 2 weeks, and continuously detect the glucose concentration in the serum after treatment for 3 days.Result: After two week, the healing effective rate of wound surface is 33% for mild,17% for moderate, 0% for severe in group I. The healing effective rate is 56% for mild,67% for moderate, 50% for severe in group II. The effective rate of group II is markedly higher than the group I(P<0.01). The effective-time for mild of group I is 27.1±3.30 days, and group II is 19.7±3.12 days. The effective-time for moderate of group I is 34.2±3.06 days, and group II is 27.5±2.81 days. The effective-time for severe of group I is 40.8±5.12 days, and group II is 31.0±2.94 days. The effective-time and cure-time of group II is markedly shorter than the group I(P<0.01). The maximal dose of group II is 84U insulin, and no patients glucose concentration was found after treatment 1h.Conclusions: using insulin can effectively shorten the cure-time of the chronic traumatic. The possible mechanism is insulin can accelerate the oxidation and decompossion of the glucose, the transport of the fatty acid and amino acid, and the decompossion of the protein, accelerate the organs to use glucose and anabolism of fat, inhibit the decompossion of the protein, these are useful for the cure of the trauma. The other possible mechanism is IGF(insulin growth factor) can accelerate the growth of the fibroblast, and accelerate the surface vesselization and epitheliumlization.
