| Objective: Through the study on the expression and their clinical significance ofco-stimulatory molecules CD40, PD-L1, B7-H3, CD3 and cytokine IL-6 in the skin lesions of neurodermatitis patients, in combination with the pathological changes in the skin lesions, the immunolopathological mechanism of neurodermatitis were investigated in order to find the new target molecule of immune intervention.Mathods: The expression of CD40, PD-L1, B7-H3, CD3 and IL-6 was detected in theskin lesions of neurodermatitis patients with the immunohistochemistry in combination with pathological observation compared with normal human skin from plastic surgery. The relationship between these immuno-molecules, clinical-typing, inflammatory cell infiltration and prognosis were also analyzed by statistically.Result: (1) The expression of B7-H3 on the lesions with 1-2 year case history withsevere inflammatory cell infiltration was upregulated significantly compared with normal skin tissues. However, B7-H3 expression had no relation to clinical stage. ( 2 ) The PD-L1 expression became higher with the course of disease history which had positive correlation with inflammatory cell infiltration. (3) CD40 expression were higher on epidermal tissues of skin lesion than that on normal epidermal keratinocytes, especially at early clinical stage less than one year and between 1 -2 year. The expression of CD40 molecule on dermal tissues was higher on early stage and tissues with light inflammatory cell infiltration than that on normal tissues. Its expression became higher with the course of disease history. But its expression became lower when the clinical stage was more than 3 year.
