| Objective: .To observe expression of WT1 gene in the different subtypes of MDS. To determine whether pattern of WT1 gene expression is a useful marker for establishing prognosis in patients with primary RA and tracking disease progression in patients with MDS.Methods: (1) 45 patients with MDS (18 patients with RA, 16 patients with RAEB, 11 patients with RAEB-t) derived from the Second Hospital of Shanxi Medical University from 2000 to 2003: including 24 men and 21 women, Its median age is 43. Diagnosis was made by the FAB. In 18 patients with RA, 13 patients with primary RA were treated nothing; The control group is 10 normal bone marrow smear storing more than 3 years, including 3men , 7women . Its median age is 40. By WHO standard classify RA patients again: 6 WHO-RA, 12 RCMD.(2)RNA were extracted from bone marrow smears, the WT1 expression were tested by FQ-RT-PCR . The WT1 transcripts obtained were normalized with respect to the expression level ofβ-actin. And expressed as WT1/β-actin. (3)Date was analysed by spss statistical software.Results:(1)The median number of WT1 expression in normal control was 2.75×10-6±20.0 ,and RA was 3.87×10-5±30.2, RAEB was1.06×10-3±4.707, RAEB-t was 1.35×10-2±6.100.The degree of WT1expression was highly correlated with type of MDS, and increased during disease progression. (2) A significant correlation was found between WT1 expression levels and blast cell percentage. (3)The WT1 expression level of the13 primary RA patients were high remarkably the normal control, 5 primary RA patients developed into RAEB/RAEB-t in half year.(4) 7 RA developed into RAEB/RAEB-t in half year for reviewing analysis disease progress of 18 RA patients , the WT1 expression of 7 RA were high remarkably11 RA which were stable in half year ; The WT1 expression level increased more a logarithm grade when a RA patient developed into RAEB, the fluctuation of WT1 expression level was less a logarithm grade during RA.(5) The WT1 transcription level of RCMD group high obviously WHO-RA according to WHO typing..Conclusion (1) This technique may be useful as a general approach in evaluating archival hematologic specimens for the expression of WT1 gene products with FQ-RT-PCR.(2) WT1 gene is a useful marker for assessment in MDS patients. (3)It pointed out may diagnosis RA if the WT1 expression level higher two logarithm grade than the mean of normal control for patients which didn,t diagnose by morphology. It may point out disease progress of MDS if the WT1 expression level increased more a logarithm grade recently. It needed confirm further because the case was limited. (4) There were different between the WT1 expression level of RCMD and WHO-RA by WHO typing. RCMD group which transformed into RAEB and RAEB-t was high WHO-RA group. |
