| Objectives:1. To study the applied anatomy of reversed island pedicled sural flap in the posterior leg in order to facilitate the operating procedure.2. To summarize the surgical technique of reversed island pedicled sural flap to enhance the curative effect and to probe into its significance.Methods:1. The anatomical observation and measurement of the relevant vessels and nerve were performed in 15 cadavers. Incision:Two round lines passing through fibular head and lateral malleolar were drawn. Then the skin, fascia and muscular membrane were reflected. A vertical line was drawn in the middle of the posterior leg and the skin with fascia was reflected to observed the anastomosed branches of the sural artery, the relationship of position between small saphenous vein and and sural nerve were measured with the use of vernier calipers under gross vision and simple microscope.2. The perforated branches of relevant vessels were detected in distal posterolateral of lower leg of healthy adults by using doppler. The position was measured from the lateral malleolar.3. The reversed island pedicled flaps were summarized in 49 patients. Close Observation of the vessels and nerves were also performed at the same time.4.
