Objective To investigate the curative effect of the operation on the elderly with degenerative lumbar spinal disorder, indications of treatment, principle of spinal internal fixation, choice of fixation and fusion to lumbar segment.Methods The clinical datas of 146 patients with degenerative lumbar disorder were collected from 2003 to 2006. All patients were divided into two groups, the operation group and control group.The operation group was treated with posterior decompression, pedical screw system fixation and bone grafting lumbar fusion. The control group was treated with expectant treatment They were followed up at 3,6,12,24 months after treatment. Clinical function of pre and post-treatment of all the cases were determined by using JOA score. The change of X-ray image were observed at the same time. The JOA score between the operation group and control group was compared.Result JOA score was significantly improved post operation in the operation group. Comparing to the control group, JOA score was significantly improved post operation in the operation group. While there is not a significant improvement for JOA score post treatment in the control group. The fusion rate was 100% in the operation group 1-2 years after operation, without breakage of the intermaterial and displacement. The final intervertebral height recovered after operation according x-ray examination of the lumber spinal. In the cases of lumber spinal instability, the ROM(range of motion) of lumber spinal diminished(<3° ).
