Objective Acute respiratory distress syndrome (ARDS ) is the most critical disease and seriously endangers children's life in pediatrics .It is the hot topic among the scholar's study in recent years ,as the pathogenesis is complicated and the mortalities is still high. Since the mechanical ventilator used to treat ARDS, the mortalities had decreased from 90% to 70%. The research report which made by the national children epidemiology cooperation showed morbidity 1.42%(PICU) and mortality 62.9% in 2005.Now lots of scholars concentrate on the mortality of ARDS . Permissive hypercapnia ventilation (PHV) is a protective strategy on the treatment ARDS , as it can avoid the high ventilation parameters ,it has been applied as a good method to treat ARDS in adult. However, the therapeutic effect of PHV on ARDS is still uncertain in children .There are lots of disputes on the indication of adaptation, complications of prevention and if it is true to decrease mortality ,in spite of certain therapeutic effects reported. Recently, it's believed that the study of PHV on ARDS is very necessary as it still on clinical trials. The main purpose of the study is to explore the values (ventilation time , complicated incidences and mortalities)of PHV on the treatment of ARDS .Subjects and methods 1 .Observing subjects: Twenty-seven ARDS patients were enrolled in this study from March 1996 to December 2005. They were diagnosed according to the diagnostic criteria of ARDS which was modified by the American -European Consensus Conference in 1994 .The patients were divided into two groups : 15 cases in the control group and 12 cases in the treatment group.2. Methods: All patients chosen were intubated via nasal and ventilated by mechanical ventilators.
