| Clinically,sphincter of Oddi dysfunction (SOD) was the main cause of biliary pain after cholecystectomy, because most of them had no structural changes, it was supposed that Sphincter of Oddi (SO) dyskinesia be the main cause. SO is a complex, valve-like, neuromuscular structure located at the junction of the bile and pancreatic ducts with the duodenum,several neural and hormonal factors are known to mediate SO motility. The main functions of the SO are to regulate the flow of bile and pancreatic juice into the duodenum and to prevent the reflux of duodenal contents into the biliary-pancreatic systems. Among these neural and hormonal mediation, Cholecystokinin (CCK) is a major hormone physiologically regulating the tone and motility of the biliary system, CCK nomally inhibits SO activity in humans and dogs but stimulates SO under various circumstances, which is known as a paradoxical response. In addition to these hormonal factors, several extrinsic nerves, such as the vagal and splanchnic nerves, maight contol the SO activity. Resent reports also shown that biliary pressure directly influence the SO motility, and the mechanism might be the reflexes between SO and the gall bladder or bile duct. The hypertention of gall bladder and/or common bile duct inhibited the SO activity through local neural reflex, which falicitate the flow of bile across the SO into duodenum. The partial obstruction of the common bile duct is seen rather frequently in human, such as cholelithiasis, stenosis of the common bile duct (CBD) and so on, most of them accompanied by biliary hypertention which should affect the motility of SO. The motility change of SO and the role of gallbladder in the mediation of SO motility during the process of common bile duct stricture have not been completely understood.
