Background and Objective:Rheumatic valvular disease is one of the main heart diseases in China. It still has a high morbility and mortality. Patients with severe heart failure have frequent occurrence of anaemia and have a worse prognosis.In this preliminary study,the clinical significance of anaemia in patients with rheumatic disease was analysed,and the mechanism of anaemia was explored.1. To analyze the cases undergoing valve replacement operation, observe the incidence rate and disposition in patients with rheumatic heart disease(RHD), and discuss the main influence of anaemia on the patients'postoperative convalescence and therapy.2. To observe the perioperative changes of reticulocyte(Ret) count, plasma free hemoglobin(FHb), serum erythropoietin(EPO), Beta2-microglobulin(β2-MG), and discuss the potential reasons and mechanism in patients with RHD.Methods:In the first part of the study, 321 cases were analyzed, in which 264 cases were non- anaemia and 56 cases were anaemia. General information, diagnosis, perioperative haematoglobin, the volume of blood transfusion and the procedures were observed. The data were analyzed statistically.In the second part, 20 patients with RHD were divided into two groups: anaemia group (n=8) and nonanaemia group (n=12). Serum erythropoietin(EPO) was determined with Enzyme-Linked-Immuno-sorbent Assay (ELISA) before operation, 1day, 3 days, 5 days, 7 days and 9 days after operation. Reticulocyte count, plasma free hemoglobin, serum erythropoietin, Beta2-microglobulin were also determined at the same time points. The perioperative changes of haemoglobin(Hb), blood volume were observed. The data were analyzed statistically.
