Objective: Associatied with the research of our department about the pathological characteristics and the regulation of the syndrome in different pathological stages of age-related macular degeneration, directed towards exudative (moist) type of age-related macular degeneration at early and middle stage, we used dispersing stagnate liver and regulating spleen as the main methods of treatment and cooperated with oral general western drugs, therefore, it can evaluate the improvement of quality of life(QOL) and clinical efficacy of exudative (moist) type of age-related macular degeneration at early and middle stage more accurately and completely, treated with Combination of Chinese Traditional and Western Medicine. Method: Using a prospective randomized comparison method, total of 65 subjects fulfilled the criteria for its diagnosis. The patients were randomly divided into two groups, that is, the treated group and the control group. The control group was treated with general drugs as vitamin C and E, inosine po. Excepting the general drugs, the treated group accompanied with Chinese medicine(using dispersing stagnate liver and regulating spleen together with addition and subtraction methods).Using Chaihu Shugan Powder and Sijunzi Decoction with plus or minus. The coures of treatment is 4-8weeks. Observations were made before and after treatment on the body of vision, ocular fundus and symptomatic improvement of TCM. At the same time, the generic instrument SF-36 and special instrument the scale of life quality about Chinese patients with visual impairment were separately used to measure QOL before and after treatment.Result: 1. Vision and Visual function: Treated with Combination of Chinese Traditional and Western Medicine, the patient's vision can't be improved dramatically, it has no significant difference before and after treatment(P>0.05). But the patient's vision of exudative AMD(at early stage of the treated group) can be dramatically improved after treatment. Compared with the control group at the same stage or the treated group at middle stage, it has significant difference(P<0.05). The result of "symptom and visual function" scale displays that it is remarkably enchanced by Traditional Chinese Medicine together with Western Medicine to cure. Compared with the control group, it has significant difference(P<0.05). The result of the treated group at early stage is higher than that of the control group in this field after treatment. 2. Clinical efficacy andSymptom efficacy: Clinical efficacy of the treated group and the control group is 56.92% and 41.38% separately. The clinical efficacy has significant difference(P<0.05). At the aspect of symptom efficacy, the efficacy of the treated group is also better than the control group. The traditional Chinese symptom efficacy of the two groups are 68.57% and 50%(P<0.05). Otherwise the clinical efficacy and traditional Chinese symptom efficacy indicate the treated group is better than the control group in exudative AMD patients at early stage, but it has no significant difference in exudative AMD patients at middle stage. 3. Quality of life: (1) Treating with Combination of Chinese Traditional and Western Medicine can improve life quality of exudative AMD at early and middle stage, especially in the scale of role emotional, mental psychology and social functioning(P<0.05). After treatment, the QOL of exudative AMD patients at different stage indicate that the treated group and the control group have no significant difference (P<0.05). Only two scale's result displays that early stage is higher than middle stage (P<0.05). (2) The level of vision is more poor, the result of scale(symptom and visual function, somatic function, physical functioning, role emotional )is more lower. Traditional Chinese Medicine together with Western Medicine can improve the exudative AMD patient's life quality result in different visual impairment groups. The improvement of QOL in the group of vision<0.1 is better than that in the group of vision > 0.1 (in the treated group) .Conclusion: 1. Treating with Combination of Chinese Traditional and Western Medicine can improve the visual function of exudative AMD patient at early and middle stage in some cases, especially at the early stage. 2. The clinical efficacy and the traditional Chinese symptom efficacy of the treated group are 56.92% and 68.57% separately. The efficacy of treated group is obviously better than the control group at early stage of exudative AMD, but it has no significant difference in exudative AMD at middle stage. 3. Treating with Combination of Chinese Traditional and Western Medicine can improve life quality of exudative AMD at early and middle stage, especially in the scale role emotional, mental and psychology, social functioning. There is no significant difference in improving QOL between the exudative AMD patient at different stage in the treated group. 4. It has some linear relationship between the life quality and the degree of visual impairment about exudative AMD patient. The poorer vision is, the lower result of life quality will be. The improvement of QOL in the group of vision<0.1 is better than that in the group of vision^O.l treated with Combination of Chinese Traditional and Western Medicine.
