Objective To provide the theory of the nerve/inflammation mechanism of IBS and approach the affect of infection on IBS , we determined the SP nerve fiber,IL—2 and IFN-γin the descending colon and rectal mucosa, analyzed the cell immunity mechanism and approached the nerves and the Th1 correlated cytokines in IBS patients'.Methods 77 IBS patients(29 men and 48 women)were divided two groups:Diarrheaed -IBS(D-IBS) and Constipated -IBS(C-IBS), and 30 healthy people (12 men and 18 women)were the controlled group. We clipped the mucosa of the descending colon and the rectum by biopsy forceps, applied the PV—6001 technology to analyze the the SP nerve fiber,IL—2 and IFN-γ. Results (1) The expressions of IFN-γin the desceding colon mucosa and rectum of D-IBS patients that were 62.22%,66.67% objectively were significantly higher than those in controlled group (P<0.01) , and higher than those in C-IBS patients. The expressions of IL—2 in the desceding colon mucosa and rectum that were 60.00%,55.56% objectively were significantly higher than those in controlled group (P<0.01) , and higher than those in C-IBS patients. (2) The SP fiber stress and fiber area in the descending colon mucosa of D-IBS patients were 14.05±1.92, 2835.85±489.31,that were significantly higher than those in controlled group (P<0.01) .The SP fiber stress and fiber area in the rectum mucosa were 13.98±2.05,2790.56±502.76, that were significantly higher than those in controlled group (P<0.01) . The SP fiber stress and fiber area in the descending colon mucosa of C-IBS patients were 12.85±2.48,2587.34±445.06, that were higher than those in controlled group (P<0.05 ). The SP fiber stress and fiber area in the rectum mucosa were 11.99±2.34,2616.21±498.87, that were higher than those in controlled group (P<0.05) . The SP fiber stress and fiber area in the descending colon and the rectum mucosa of D-IBS patients were respectively higher than that of C-IBS (P<0.05) . (3) The SP fiber stress and fiber area in the descending colon mucosa of PI-IBS in the D-IBS patients were 14.97±1.78,2882.98±450.22, that were higher than those in non- PI-IBS group (P<0.05) . The SP fiber stress and fiber area in the rectum mucosa of PI-IBS in the D-IBS patients were 13.98±2.05,2850.56±523.76, that were higher than those in non-pIBS group (P<0.05) . (4) The expressions of IFN-γin the desce(s) o(?)on mucosa and rectum of the PI-IBS group of D-IBS patients were 85.00%,80.00%,that were were significantly higher than those in controlled group (P<0.01). The expressions of IL—2 in the desceding colon mucosa and rectum that were 80.00 %,75.00% objectively were significantly higher than those in controlled group(P<0.01).(5)The expressions of the SP fiber stress in the descending colon mucosa in the IL—2 and IFN-γsubgroups of pIBS patients were 16.07±3.24, 15.32±3.67,that were significantly higher than those in controlled group(P<0.01). The expressions of the SP fiber stress in the rectum mucosa in the IL—2 and IFN-γ, subgroups of PI-IBS patients were 16.07±3.24, 15.32±3.67,that were significantly higher than those in controlled group (P<0.01).Conclusions (1)The expressions of SP fiber stress and area increased in IBS patients' descending colon and rectum mocosa. In D-IBS, the expressions of SP fiber stress and area in PI-IBS patients' descending colon and rectum mocosa were higher than that of the non- PI-IBS patients. (2) The expressions of IL—2 and IFN-γin the descending colon and rectal mucosa in D-IBS patients increased. The expressions of the SP fiber stress in the descending colon mucosa in the IL—2 and IFN-γsubgroups of D-IBS patients increased obviously. (3) The infection could promote the expressions of SP, which then could precipitate the up-regulation of IL—2 and IFN-γ, so that the cell immune reaction were actived, and induced the IBS.Postgraduate student: JuHui(Gastroenterology)Directed By Prof. Wei Liangzhou...
