ObjectiveThe kidney is the most sensitive targeted organ to the obstructive jaundice. Previously,people have done considerable work to illustrate the mechanism of the renal defunction during the OJ perioprative period. These theories mainly approached the external factors which may lead to the renal defunction;inversely,it is almost unclear whether the renal de-function is resulted from the internal factors during the OJ perioprative period. The aquaporins in the kidney are just the small molecular proteins which are located in the kidney and many documents indicated that the aquaporins change in various renal diseases which are resulted from different causes. However, there is still few reports about the relation between the renal defunction and the AQPs during perioprative period of the obstructive jaundice up to now. The present study primarily approach the change of the renal AQP3 after the obstructive jaundice is released.MethodsThe study were performed in adult male Wistar rats which sum to 40. Firstly,we established the model of OJ and after a week the obstruction is released. The experimental animals was randomly divided into the following four groups which were based on the time when the samples were removed: OJ0h (The samples were immediately removed after OJ were released,which equaled to the state in which OJ wasn't released .)^OJ24h(The samples were removed at the 24th hour after the opration was taken) ^OJ72h(at the 72th hour after the opration) ^OJlw(the 7th day after the opration ). Then we calculated biochemistrical indexes in the blood(such as TBIL^UREA ^ ALT,etc. ),measured the expression levels of the AQP3mRNA by the Rt — PCR technology and the expression levels of the AQP3 protein was measured through the Western blotting. At last,the data were analysed by statistical method.Results1. The blood biochemistry showed that the TBIL and ALT significantly deceased after the opration(p<0. 05),which proved the opration was effective. At the same time, the UREA significantly increased at the 24th after the opration (p0. 05), but its levels is significantly downregulated at the 7th day after the opra-tion(p<0. 05)3. Semiquantitative immunoblotting( Western blotting) showed that the change of the expression levels of the AQP3 protein wasn't significant within 3 days after the opration(p>0. 05). In contrast, its expression levels of the protein is significantly upregulated at the 7th day after the opra-tion(p<0. 05).DiscussionOJ is a kind of common disease in hepatobiliary surgery. Many complications (such as septicaemia ^ gastrointestinal hemorrhage N wound delayed intention^ ARF, and so on) will happen during the perioprative period. The kidney is the most sensitive targeted organ to OJ. In clinic, the renal function of all patients with OJ is damaged in different digreeand the operative attack often leads to more defunctional kidney,even renal failure which is the main reason that results in patients' death after the opration.People have various opinions on the mechanisms of the renal defunc-tion during the perioprative period. Presently, main theories are at follows :ischaemia — reperfusion injury, bilirubinic direct injury to the renal tubule, intestinal barrier injury leading to increased endotoxin in blood and considerable oxygen free radical in blood,etc.. These theories mainly approached the external factors which may lead to the renal defunction;inversely, it is almost unclear whether the renal def unction is resulted from the internal factors during the OJ perioprative period. The aquapor-ins in the kidney are just the small molecular proteins which are located in the kidney and many documents indicated that the aquaporins are changed in various renal diseases which are resulted from different causes. However, few studies involved in the changes of the renal AQPs after obstructive jaundice surgery all over the world.The present experiment showed the renal function decrease during short period after the obstructive jaundice surgery and has been significantly improved at the 7th day . Meanwhile, the expression levels of the AQP3mRNA is downregulated when the renal function was improved,but the expression levels of the AQP3 protein is decreased;in turn, so is it. Thus,we conjecture that the paradoxical phenomenon in our experiment may be resulted from certain mechanism called feedback, i. e,the AQP3 protein is still able to be synthetized in certain degree when kidney is injured ,however, the synthesis of the protein suddenly increased to make the genie expression levels of AQP3 downregulate through feedback as the renal function deteriorate. Obviously, additional studies will be needed to manifest the essential mechanisms of the phenomenon.In our previous experiment, it has proved that the clinical symptoms of the renal defunction just emerge till the 14th day after the common bile duct of rats was ligged. Associated with the present experiment, the expression levels of the AQP3 protein has decreased before the clinicalsymptoms of the renal defunction didn't appearced during the earlier obstructive period. So,we can earlier diagnose and preclude the renal defunction by detecting the expression levels of the AQP3 protein.Conclusions1. The expression levels of the AQP3mRNA is downregulated as the renal function improves after obstructive jaundice surgery.2. The expression levels of the AQP3 protein is upregulated as the renal function improves after obstructive jaundice surgery.3. We can earlier diagnose and preclude the renal defunction by detecting the expression levels of the AQP3 protein.
