In this study, the severe acute pancreatitis (SAP ) specific EN formula was composed based on the rationale of the nutritional mechanism of the disease, and the alanyl- glutmine (Gln )dipeptide was added into the formula. This study was cut in the combining site of clinical nutrition pharmacology and immunology to probe the effect of Gln in the special formula in SAP rat form several aspects including mucous barrier, immunological barrier and systemic inflammatory responses. During this study, we deployed immunology and molecular biology means to study. This study result show that using glutamine in enteral nutrition (EN+Gln) could prevent gus bacterial translocation in SAP rat and therefore to decrease secondary infection. The results suggested that the EN+Gln was better in the ability of modulating the balance of pro- and anti- inflammatory response than EN without Gln and parenteral nutrition group. EN+Gln could increase the expression of occludin protein and the gus barrier fuction of EN+Gln could be better than EN without Gln. This study provides SAP clinical research with new method for prevention and cure SAP secondary infection, lower mortality rate, improve prognosis.
