Study On The Quality Of Life And Influential Factors Of Patients After Pneumonectomy

Objective:To study on the quality of life (QOL) and influential factors of patients after pneumonectomy, and to discuss the scientific bases for improving QOL of the patients after operation .Methods: 190 patients after pneumonectomy (77 cases from Shandong Provicial Hospital, 75 cases from Qilu Hospital, 38 cases from Shandong Provincial Chest Hospital) were investigated with SF-36 at 6-18 months after operation . In corresponding factors , some were confirmed or complemented by means of Questionnaire,telephone,others including pathology, pulmonary function ,smoking indexes and etc were acquired through medical records.All of the information and clinical data were analyzed by t-test, ANOVA, rank sum test(Kruskal-Wallis test) and stepwise regression analysis. Results: Single factor analysis indicates:the influential factors including the nature of disease,chemotherapy or radiotherapy after operation,symptoms of pain or cough after pneumonectomy and the living environment, are very important to multi-dimental QOL. However, some influential factors including sex,age,smoking,inflammatory sites,types and stages of lung cancer are little significant to multi-dimental QOL. Stepwise regression shows:In factors above,the nature of disease,postoperative complications, chemotherapy or radiotherapy after pneumonectomy are the most important to the dimentions of QOL.but all of the others are little. Conclusion:(l) QOL of patients after pneumonectomy are better than those who had no operations. Postoperative respiratory function exercises can improve pulmonary function and QOL of patients. (2)It is very significant to strengthen the process of scientific research and comprehensive therapy,which can improve prognosis judyement and QOL ,also alleviate psychological pressure of patients .(3)The therapy of postoperative complications and side-effect of chemotherapy or radiotherapy after pneumonectomycan alleviate psychological pressure of patients and improve apparently clinic symptoms and function of body of patients. (4)Behavioral and psychological intervention can improve psychological state of patients after pneumonectomy effectively.Study the role of psychological factor on disease and disease severity in treatment of patients.
