Objective: To evaluate the relationship between hypertension with ischemic stroke and ambulatory pulse pressure, carotid atherosclerosis. Methods: 116patients with essential hypertension were divided into two groups: 61 patients with ischemic stroke and 55 patients without ischemic stroke. ABP (ambulatory blood pressure), Carotid altrasound and cerebral CT(or MRI) were examined in all subjects, The date of age, smoking, blood lipoprotein et al, were obtained for each of the patients. Results: the age, daytime SBP, 24h SBP, daytime PP, nighttimePP, 24hPP, carotid IMT of patients with ischemic stroke are significantly higher than those without ischemic stroke(p<0.05), the patient with ischemic stroke have more carotid plaque than those without stroke(p < 0.05). Conclusions: Increased ambulatory pulse pressure and carotid atherosclerosis may have positive effect on ischemic stroke in essential hypertension.
