The Effect Of Lidocaine On Inflammatory Mediators And Myocardial Enzyme In The Patients Serum Undergoing Off-pump Coronary Artery Bypass Grafting

ObjectiveWe are surposed to determine whether lidocaine can influence the concentration of serum inflammatory mediators and cardiac enzyme for the patients undergoing off-pump coronary artery bypass graft we choose tumor nerosis factor-a(TNF-a), interleukin-8(IL-8), superoxide dismutase(SOD), malindialdehyde(MDA), cardiac troponin I(cTnI), the MB isoenzyme creatine kinase (CK-MB), myoglobin(MYO) as the markers.Methods: 20 patients underging OPCAB are randomly divided into 2 groups:the control group and the lidocaine group.There are 10 patients in each one. In the lidocaine group,a bolus of lidocaine 2mg/kg is given during induction. An infusion of lidocaine 2mg/kg/h is followed up for maintenance. For the control group,the same amount of normal saline is given.Blood samples are taken from the internal jugular vein at the following poits: preoperative, the time just after heparinization, at the end of operation, 24h after operation. TNF-a, IL-8 and cardiac enzyme are measured by ELISA. SOD, MDA are measured by spectrophotometric method. Record the information about hemodynamics, intraoperative complications, early postoperative clinical information, the duration that patients stay in ICU and hospital.The time points measuring cardiac enzyme :preoperative, at the end of operation, 24 and 48h after operation.Results:â‘ TNF-a ,EL-8 rises significiently in both groups comparing with preoperative(P<0.01).There is significently difference between groups(P<0.01 or 0.05).â‘¡SOD keeps steady at different points in both groups(P>0.05).MDA in control group rises significently at 24 hours after operation(P<0.01). There is significently difference between groups(P< 0.05). â‘¢ Comparing with preoperative ,CK-MB, cTnl risein control group significiently (P<0.01)24h and 48h after operation.they rise too in lidocaine group, but there was no significient diference between points(P>0.05). There is significently diference between groups(P<0.01 or 0.05).MYO rises but there is no significently difference in the two groups(P>0.05) ?Patients in the lidocaine group stayed for less time in ICU and hospital(P<0.05).Conclusion.a continuous intravenous infusin of lidocaine can significently decrease inflammatory mediators and myocardial enzyme and has myocardial protective effect on patients undergoing OPCAB.
