The trunk is an important organ of the human body, its main function is of movement, coordinate and balanced function. Lower back pain is a kind of clinical frequently-occurring disease and common disease, which influence patient's life quality and labor capacity seriously. The mechanics change of the trunk has very important role in the occurrence and aggravation of lower back pain. To study the mechanical characteristics can help us to understand the cause of lower back pain and to prevent its occurrence effectivelyThis study comprises three parts.Part I: Study on the validity of isokinetic test of the trunk musclesBackground: Isokinetic muscle test is a more exact value than manual muscle test (MMT), especially more convenient and practical in the trunk evaluation. But as a kind of method to evaluate the trunk muscles, validity must be confirmed first.Objectives: To study the validity of isokinetic test of the trunk muscles, and to analyze the factors which may influence the isokinetic trunk test. To establish the foundation for further investigating.Methods: 20 volunteers are tested twice by the same person with the same procedure, the same method within the same week. ICC was analyzed about their PT, PT/BW, TW, TW/BW and AP.Results: The validity of test-retest of 60°/ s and 180°/ s is satisfied, and the validity of extension of 240°/ s is also satisfied but the flexion is inaccurate.Conclusion: The test-retest validity of isokinetic trunk muscles is good when using our isokinetic system. Our results suggest that low and medium velocity should be chosen when test. And the test results in high velocity maybe inaccurate.Part II: Influence of velocity on isokinetic test of the trunk musclesBackground: The trunk strength is affected by many kinds of factors, such as sex, age, height, weight, sports level, etc.. But the test velocity may play the important role during isokinetic trunk test.Objectives: To analyze the results of isokinetic trunk test of different groups. And to drew the best velocity parameter for the test. To provide scientific basis for the understanding the mechanical characteristics of lower back pain.Method: 20 male health volunteers were tested at 60°/ s, 180°/ s and 2407s. The different results in different groups were analyzed, and the best velocity to test was confirmed.Results: PT, PT/BW decreased with the velocity increasing, but AP reached its peak value during the medium velocity. There is no statistical significance to observe AP during high velocity.Conclusion: The influence of the test velocity on isokinetic muscular strength is obvious, it is more reliable to choose the low-speed while testing and training.Part III: Isokinetic evaluation on the lumbar disc herniationBackground: The mechanics change of the trunk maybe the risk factor for the onset of LDH. If it is true, to correct this kind of change can offer us a new thinking for the treatment of LDH.Objectives: To compare the difference of isokinetic results of LDH and control group. To confirm the mechanics change of the trunk for LDH patients, and its role on the occurrence and aggravation. To offer scientific basis for treating effectively.Methods: PT, PT/BW, TPT, T@0.2, TW, AP and F/E were tested for LDH and control group. The differences between the two groups were analyzed to confirm the mechanics change between the two groupsResults: All results of LDH were different from the control. The difference may cause back pain, cause muscle spasm, cause ligament freeze, andcause the damage of the intervertebral disc and the structure around it.Conclusion: It is definite that the patient of LDH has an obvious change of trunk mechanics, this kind of change may play an important role for the occurrence and aggravation of LDH. The suitable and pointed solution should be made under the accurate evaluating this kind of change.
