Objects To observe the effects of phototherapy on blood endothelin(ET) and nitric oxide(NO) of preterm infants,elaborate the blood flow dynamics unstable mechanism of preterm infants when phototherapy and instruct the correct opplication of phototherapy in preterm infants.Methods 64 preterm infants with unconjugated hyperbilirubinaemia were divided into the gestational age>32 weeks group (n=31),the gestational age≤32 weeks group (n=33). 26 term infants with unconjugated hyperbilirubinaemia were in control group. Offer continuous phototherapy for 24 hours. Measure the blood ET,NO value,the heart rate,the breath,the average artery pressure value of each group before, 12h and 24h into phototherapy.Observe the side effect which occurs after photo- therapy completing.Results (1) The blood ET levels measured 24h into phototherapy were significant higher than the levels measured before phototherapy in gestational age ≤32w group(P<0.05).(2) The blood NO levels measured 12h and 24h into phototherapy were both significant higher than the levels measured before phototherapy in gesta- tional age ≤32w group(P<0.05). In gestational age>32 weeks group the blood NO levels measured 24h into phototherapy were significant higher than the levels measured before phototherapy(P<0.05). (3) The blood NO/ET value measured 24h into phototherapy were significant higher than that before phototherapy in the gestational age ≤32w group(P<0.05).(4) In gestational age≤32 w group the heart rate increase and the average artery pressure drops during phototherapy. They measured 24h into phototherapy were both significant difference with the values measured before phototherapy (p<0.05). In gestational age≤32w group ,the NO/ET value and the average artery pressure present the negative relevance.There are more requent apneao of newborns in the gestational age≤32w group compare with other two groups.(5) Each group of troubles have side effect occurrence such as fever,the skinrash,the diarrhea and the low calcium blood sickness.The low calcium blood sickness formation rate was significant higher in the gestational age≤32w group than that of other groups(P<0.01). The low calcium blood sickness formation rates were significant higher after phototherapy than formation rates before phototherapy in all groups (p<0.05). Conclusion Phototherapy does affect the blood ET,No levels of the gesta- tional age ^ 32w group .Along with the phototherapy time lengthens,the NO/ET value increase.The NO/ET value and the average artery pressure present the negative relevance. This prompt us in premature infant especially exetremely low-birth weight newborn long-time phototherapy is dangers. Premature infant especially exetremely low-birth weight newborn and critical one should be cautious when carried on phototherapy in order to prevent the phototherapy illness complication occure.
