Experimental Study About Therapy Of Yishen Huoxue Decoction On Chronic Renal Failure

Objective: Glomerulosclerosis is the common result of glomerular injury induced by all kinds of causes and is one of the pathologic bases of chronic renal failure. As there's not any special treatment to chronic renal failure, we observed the effect of Yishen Huoxue decoction on 5/6 nephrectomized rat model, and tested the items: PCNA and FN that can reflect the increase of cells and the accumulation of extracellular matrix and the expression of TGF-β1, CTGF and PAI-1 in glomeruli that can accelerate the progression of glomerulosclerosis .At last we discussed the protection and mechanisms of Yishen Huoxue decoction in preventing and treating chronic renal failure, and we hope it may be a foundation for clinic using.Methods: The glomerulosclerosis model was established by 5/6 nephrectomy and the rats were assigned to normal control group (group A), modle group (group B), Chinese traditional medecine Yishen Huoxue decoction group (group C ) and west medicine Lotensin treatment group (group D ).Urin protein was measured at the 6th and 12th week .Blood urea nitrogen,serum creatinine ,albumin and haemoglobin were measured at the 12th week after operation. Renal pathology changes were evaluated at the 12th week with light microscope .Immunohischemistry was used to examine the expression of FN and PCNA in glomeruli at the 12th week.RT-PCR was used to examine the expression of TGF- β1, CTGF, PAI-1 at the 12th week after operation.Results: At the end of the 6th week, the levels of urin protein of 24 hours in group B, C and D were already significantly higher than that in group A(P<0.01), and at the end of the 12th week, the levels of urin protein of 24 hours in group B, C and D increased even higher than that in group A(P<0.01). At the end of the 6th and 12th week, the levels in group C and D were significantly lower than that in group B(P<0.01). At the end of the 6th week, the levels of urin protein of 24 hours in group C and D decreased separately 26.08%, 27.29% than that in group B, and at the end of the 12th week, the levels in group C and D decreased separately 28.92%, 29.19% than that in group B, but there is no significant difference in group C and D (P >0.05) at the end of the 6th and 12th week. At the end of the 12th week, the levels of blood urea nitrogen,serum creatinine in group B, C and D were significantly higher than those in group A(P<0.01), and the levels in group C and D were significantly lower than in group B(P<0.01), but there is no significant difference in group C and D (P >0.05). The levels of albumin and haemoglobin in every group at the end of the 12th week were similar(P >0.05). The renal pathology showed that the percentages of the area of mesangial division in glomeruli (%) in group A, B, C and D are 3.78±0.73 , 21.99± 6.79, 11.36 ± 2.17, 9.73±2.34 separately, and the percentage in group C and D decreased 48.34%, 55.75% separately than that in group B(P<0.05), but the target was similar in group C and D (P >0.05). Immunohistochemistry staining indicated that the expression of FN in group B( 19.12±1.27) is higher (P<0.05)than that in group A (1.03±0.26) , and the expression of FN in group C (7.81 ± 1.68) and D (7.87 ±1.64) is lower than that in group B (P<0.05). The staining of PCNA indicated that the number of the positive cells in group B is significant higher than that in group A (P <0.05), and the number in group C and D is lower than that in group B (P <0.05). But there is no significant difference in group C and D (P >0.05). RT-PCR indicated that the expression of TGF- β 1, CTGFand PAI-1 in group B is separately 6.25, 7.06, 7.22 times of that in group A(P <0.05). There were a decrease in the expression of TGF- β 1, CTGFand PAI-1 in group C and D compared to group B (P <0.05), but these targets were similar in group C and D(P>0.05).Conclusions: Yishen Huoxue decoction can obviously decrease the urin protein ,improve the renal function, and attenuate the degree of mesangial proliferation and extracellular matrix disposition of 5/6 nephrectomized rats.And it can also significantly inhibit the over-expression of FN and PCNA in glomeruli and the over-expression of cytokines as TGF- β 1, CTGF and PAI-1 with glomerulosclerosis.So its mechanisms in delaying glomerulosclerosis was found related to the inhibitory effect on the over-expression of FN ,PCNA, TGF- β 1, CTGF, PAI-1 and the alleviation of the accumulation of extracellular matrix and the increase of the number of cells in glomeruli.
