Objective: To summary the occurrence and development regulation in diabetic rats and to analyze the characteristics of Color Doppler Imaging (CDI)and clinical datas of the diabetic retinopathy (DR.) patients , and to classify DR according to CDI.Methods1. The 52 Streptozotocins- induced diabetic rats included, the weight, glucosuria, fasting blood glucose and CDI of eyes were examined at the 1, 2, 3, 4, 5, 6 months respectively. To execute the rats and extract the double eyes at different intervals, the retina of the left eyes were digested preparative and stained the period acid schiff (PAS) and right eyes were taken immunohistochemistry with NF- kBp 65 and TNF- a.2. To classify DR and macular edema (ME) on the basic of the CDI performed on 152 patients (304 eyes) ,and to compare it's relation with other classifications based on the analysis of duration of diabetes mellitus (DM), the visual acuity, manifestation of CDI imaging and results of ophthalmoscope examination.Results1. Blood flow velocity reduced, pulsatility index(PI) and resistive index( RI) increased in central retinal artery(CRA) with the progression of diabetic; blood flow velocity in the central retinal vein( CRV) increased in DM 6 months. The Diameter ofCRA(Dcra) dilated and the Diameter of CRV(Dcrv) contracted in DM 3 months and Dcrv dilated and Dcra became normal in DM 6 months.2. The pericytes nucleus was swelled and dyes shallow in DM 3 months; there are ghost capillary vessels and haemangiomas were founded at DM 4 month.3. Capillary vessels dilated and ganglion cell vacuole denaturalized at DM 3 month ; Neovascular was found at DM 6 month .4. The positive reaction of NF- kBp65 was found in the retina of control rats, while TNF- a was not found. Both were founded in diabetic rats and increasing with the development of disease.5. In 304 eyes, there were 39 eyes (12.8%) without DR, 191 eyes (62.8%) with DR I , 28 eyes (9.3%) with DRII, 46 eyes (15.5%) with DRIII. there were 67 eyes (22%) with ME, including 29 eyes (43%) with focal ME, 25 eyes (22%) with diffused ME.Conclusion1. At the early stage of diabetes, the abnormal changes of retinal blood flow can be founded before the pathological change by the circulatory parameters in CRA with CDI. Retinal vessels occlusion which formed a mass of no- perfusion areas related with the changes of MV; PI; RI ,and Dcra dilated in the early presence of high glucose. There was a relation between Dcrv' s dilation , CRV blood flow velocity increasing and the pathologic changes of diabetic retinopathy.2.The expression of NF-kBp65,TNF-a in the retina of the diabetic rats related with the damage of retina capillaries in diabetic retinopathy which aggravated ischmia and oxygen deficit of diabetic retina tissues.3.
