Objective: To study the expression of CD34,endosatin,VEGF and bFGF in scarsin different stages and different degrees of proliferation and to investigatethe relationship of proangiogenic / antiangiogenic cytokines and the roleand significance of both in the formation and development of scars, fortherapy of scar from angiogenic aspect. Methods: Different stages and different pathological types of scars were selected,which were compared with normal skin. The expression and distributioncharacter of CD34, endosatin,VEGF and bFGF in the scars was observedwith SP immunohistochemical method.. Results: Part I.Experimental study on scar tissue revascularization It is found that the main CD34 positive staining located in microvesselendothelial cell plasm of the dermis and papillar layer. Higher expressionof CD34 was detected in hypertrophic scar and keloid, without significantdifference between them, on the contrary, in non-hypertrophic scar andnormal skin, the CD34 expression was at extremely low level; Theexpression of CD34 positive staining representing microvascular density inthe scars less than 6-month duration increased compared with that in thescars of 6 to 12 months, and CD34 positive staining in the scars of 6 to 12months is higher than that of the scars over 12 months, however, theexpression of CD34 showed no significance difference between the scarsover 12 months and normal skin. Part II.Experimental study on expression of endostatin in scar tissue It is found that the endostatin positive staining mainly located inkeratinocytes plasm, especially in basal layer; low partial positive stainingin microvascular. Higher expression of endostatin was detected inhypertrophic scar and keloid, without significant difference between them,on the contrary, in non-hypertrophic scar and normal skin, the endostatinexpression was at extremely low level; The expression of endostatinpositive staining in the scars less than 6-month duration increasedcompared with that in the scars 6 to 12 months, and endostatin positivestaining in the scars of 6 to 12 months was higher than that of the scarsover 12 months, however, the expression of endostatin showed nosignificance difference between the scars over 12 months and normalskin.According to correlation test, the expression of endostatin showedsignificant correlation with VEGF,bFGF and microvascular density. Part III.Experimental study on expression of VEGF and bFGF in scar tissue It is found that the VEGF positive staining located in keratinocytesplasm, especially in basal layer; low partial positive staining inmicrovascular. It is found that the main bFGF positive stains granules located inepidermis stratum spinosum,blood vessel endothelial cells,fibroblasts ,hair follicle epithelial cells. Higher expression of VEGF was detected inhypertrophic scar and keloid, without significant difference between them,on the contrary,in non-hypertrophic scar and normal skin, the expression ofbFGF was at extremely low level; The expression of bFGF positivestaining in the scars less than 6-month duration increases compared withthat in the scars of 6 to 12 months, and bFGF positive staining in the scarsof 6 to 12 months was higher than that of the scars over 12 months,however, the expression of bFGF showed no significant difference betweenthe scars over 12 months and normal skin.
