Objective: To explore treatment of intertrochanteric fracture by DHS through anatomical research and clinical application.Methods: 126 cases of intertrochanteric fracture according to EVENS classification were treated by DHS, and fracture reduction is done under "C" arm x-ray machine or inciseon. According to TAD, all of cases divide into two groups: TAD of group 1 is less than 25 mm and TAD of group 2 is more than 25mm. Followed up for 8~24 months, the quality of fracture reduction was observed. The classif-ycation of the quality of fracture reduction: Excellent- the anatomical reduction is showed from x-ray. Good-it is showed that line of force of fracture is good but bad paratope in inboard bone cortex from x-ray. Bad-it is showed that line of force is bad and inboard bone cortex defect from x-ray.Results: Among 126 cases, group 1 is 70 cases and group 2 is 56 cases; Complications rate: group 1 is 17.1% and group 2 is 41.1%. Compared between group 1 and group 2, complications rate increased along with the reduction dropping in quality. T testwas conducted between the two groups (P<0.05) showing significant difference.Conclusion: The quality of reduction of DHS on femoral neck and femoral head has significant relativity with the treatment of inte-rtrochanteric fracture. To reduce complications, we should improve the quality of reduction under "C" arm x-ray machine and correctly grasp the position of fracture.
