[Objective] As a preliminary survey for the project of Integrated CareManagement of Diabetes Patients, it is designed to learn of the diabetes patients' knowledge, attitude and behavior about diabetes, to learn of the life quality and the influential factors of life quality, and to study the demand and utilization of health services from the patients' view, thus, the scientific evidence could be provided for instituting and applying community intervention measures, for studying the demand and meaning of the integrated care management which includes both comprehensive hospital and community health centers, also for the basis for evaluation on the effect of the integrated care management.[Methods] Two communities of Wuhou district, Chengdu, Yulin community and Shuangnan community, are chosen as study field. 110 and 74 type 2 diabetes patients from either community are sampled through convenient sampling method. The nurses from both community health centers as investigators go directly to the patients' family to interview them with the self-designed intervention questionair, which includes such four sections as basic data of patients, the KAP section, measurement of diabetic quality of life, and the section studying the demand for health services of patients.[Results] The average score of diabetic knowledge is 5.76±2.40(total 12). Diabetes patients do well in applying appropriate life and exercise style, but have a bad grasp on scientific principle of diet. The life quality of the patients is comparatively perfect, and the influential factors of life quality include whether there is a complication, the capacity of paying medical costs and the behavior score. Diabetes patients have different demand for medical institutions in differentdisease state. As for the medical services ever received by the patients, diet guidance, health education and regular physical examination rank the first three places. Patients hope to obtain diabetic knowledge mainly by joining the doctors' lectures(39.8%) and reading diabetic books(30.6%). The advices on development of community health centers advocated by patients includes enlarging the size of the institution, increasing the clinical departments related to DM, strengthening health education, etc.[Conclusion] Carrying out integrated care management of diabetes patients in communities can play an active role in standardizing the regulation on diabetes patients, instituting scientific treatment and life style for patients, and improving life quality of diabetes patients. It is necessary to set up integrated care management system for diabetes patients from both subjective and objective view.
