Hematuria is the familiar clinical symptom in urinary system disease, the hematuria in primary glomerular disease, including hematuria in naked eye, hematuria in microscope, and hematuria accompanied with protein urine. There is no certain therapy with west medicine, and the pathogenic damage of some patients is ongoing, will result in renal failure finally. Primary glomerular hematuria , in TCM, belongs to blood urine, waist pain, and emptiness syndrome, ect. The clinical symptoms of primary glomerular hematuria are different according to the different pathogenic factors. Therefore, treatment determination based on syndrome differentiation can accept good effect. According to the clinical characters, many patients have the blood stasis syndrome in TCM, and blood stasis can be an independence syndrome , also an accompanied symptom in other syndromes. This research set up contraposing the clinical effect, based on the wholism conception and treatment determination based on syndrome differentiation in TCM, the principle is eliminating blood stasis , regulating blood circulation, clearing heat and detoxifying. The research sets up the systemic clinical observation and study in primary glomerular hematuria using purifying kidney decoction, and aims at the clinical effect and approach the mechanism of this decoction. There are 78 cases in this research, all from TCM department and renal medical department of China-Japan Union hospital of Jilin University, the male are 42 cases, and the females are 38, the age of them is from 18—36, the average age is 38.5, divided into therapy group and control group in random, and there is 40 cases in therapy group, 38 in control one. All of the cases are diagnosed as primary glomerular hematuria according to the , and , and differentiated syndromes according to ,, and all the patients haven't had correlative drugs in last 2 weeks. Therapy group: purifying kidney decoction , 100ml, b.i.d oral. Basic prescription: Field Thistle, Rubia Root, Japanese Thistle, Rehmania Root, Honeysuckle Flower, Forsythia Fruit. Ifthere are symptoms of deficiency qi of spleen, such as listlessness, large tongue with teeth print, modify with Astragalus Root, Pilos Asiabell Root; if purple tongue or relapse, modify with Notoginseng powder, Peach Kernet, Safflower; if deficiency of kidney essence ,such as soreness of waist and fatigue, modify with Eucommia Bark, Dipsacus Root; if protein urine, modify with Astragalus Root, Corn Stigma, Cogongrass Rhizome( all Chinese herbs are decocted in TCM pharmacy of China-Japan Union hospital ). Control group: routine therapy with west medicine. Therapy course: 4 weeks as a one course, 2—4 courses consecutively. The result: ①The patients'urinalysis BLD, RBC decreased distinctly in therapy group after taking purifying kidney decoction, dealt with statistics( P<0.01 ), has the difference significance. ②The RBC elimination rate declined, and the abnormal red blood cells decreased in therapy group after taking the decoction, dealt with statistics( P<0.01 ), has the difference significance. ③The decoction can rectify the low serous alexin syndrome, andmodulate immunoglobin in therapy group, dealt with statistics( P<0.01, P<0.05 ), has the difference significance. ④The can also decrease the higher CRE and BUN, improve renal function, dealt with statistics( P<0.01, P<0.05 ), has the difference significance. The purifying kidney decoction takes the theory of TCM as the basic principle, has the effect of eliminating blood stasis, regulating blood circulation, clearing heat and detoxifying. In the decoction, all the herbs supplement each other to acquire perfect clinical effect. In recent research, the Chinese herbs in this decoction has the following effect to treat hematuria: 1. promote the generation of coagulant factors, enhance the concentration and movement of coagulant factors. 2. Increase the quantity of platelet, and improve the function of platelet. 3. Contract the local vessels and improve the function of the vessels, strengthen the resistance of capillary vessels, decrease the penetration of the vessels. |
