Objective To study the correlation of pulse pressure with cardiovascularand kidney's function and structure and other factor in aged essentialhypertensive patients. Methods Aged patients with essential hypertension wereenrolled into the following three groups by the mean values of pulsepressure :PP<60mmHg , 60mmHg≤PP<80mmHg and PP≥80mmHg,Bloodpressure were examined.Left ventricular end diastolic diameter(LVEDd),leftventricular end systolic diameter(LVEDd),interventricular septumthickness(IVST), left vemtricular posterior wall thickness(LVPWT), leftventricular mass index,the ratio between early diastolic peak flow velocity andartrial peak flow velocity(E/A),ejection fraction(EF),left ventricular massindex(LVMI),carotid arterial diameter(CAD),carotid artery plaque rate(CAPR),and intimal medial thickness(IMT) ,flow-mediated dilation were measured byechocardiography .Creatinine clearance rate (Ccr) , microalbuminuria(MAU),high sensitive C-reactive protein(hsCRP),homocysteine(tHcy) were tested . Therelationship between circulating levels of high-sensitivity C-reactiveprotein(hsCRP) ,homocysteine(tHcy) and pulse pressure(PP) was valued.Results In the group with PP≥80mmHg,EF,E/A, Ccr,CD and FMD weresignificantly decreased ,while LVMI,CAPR,IMT,S and MAU were increasedcompared with the other twe groups(P<0.05). A positive relationship was foundbetween PP and hsCRP(P<0.001),tHcy(P<0.05). Conclusion The increasedpulse pressure was associated with the alteration in left ventricular systolic anddiastolic function,increase in LVMI,CAPR,IMT and MAU,and decrease in Ccrin aged patients with essential hypertension. PP may be one of the risk factorsfor the organ damages in elderly essential hypertension. Increases in PP areassociated with elevated hsCRP and tHcy levels among EH of the oldaged .Inflammation response and tHcy might play an important role in thepathogenesis of essential hypertension.
