| Objective: Malignant cancer is one of the main diseases that badly do harm to human beings' health, and it is the third primary factor that causes the death of human beings in the world. Therefore, how to cure cancer has become one of the focus problems in the world. Nowadays, the admitted therapies are surgical operation, radiotherapy, drug treatment, and biotherapy, etc, but all the therapies above cause body injures in different degrees. Chemotherapy is one of the most effective therapies for cancer, but its selective inhibitory action on tumor cells is not strong, and its side effects on the whole body is huge, so many sufferers have to reduce the dosage, even give up the therapy.With the development of traditional Chinese medicine (TCM), the combination of western medicine and TCM that has the traditional characteristics of our country has become another method to cure cancer. Especially in recent years, acupuncture and moxibustion has shown its distinct advantages in increasing the curative effect of chemotherapeutics and reducing the side effect of chemotherapeutics (e.g. relieve the marrow inhibition after chemotherapy, improve the symptoms of digestive tract, enhance the immunity function of patients, and improve the life quality of patients.) It is not only an effective therapy, but also an inexpensive method.However, we also found that although the kinds of acupuncture and moxibustion are various, methods are multiple, and the differentiation of points has a great variety on the aspect of relieving the side-effect of chemotherapy, experiment that really abides to the method of scientific design, measurement, and evaluation is not too much. Furthermore, few reports have screened effective single point or prescription with significant effect. Therefore, if effective single point can be screened on the basis of abiding to the scientific methods, observe the function of the single point, and it has important meaning to the clinic and scientific research in the future.This clinical research is a prospective study, and strictly abides to the scientific method. On the basis of using western medicine to resist the side-effect of chemotherapy, take the single point of Geshu and Zusanli that has the function of strengthening the genuine qi as the main prescription, adopt electrical acupuncture to treat the side-effect of tumor chemotherapy, set up Control Group to compare, and systemically observe the effects of these two single point groups on the side-effect of chemotherapy. Develop the specific research of single point in treating the side-effect of chemotherapy, screen effective singlepoints, and illustrate the specific functions of points, so as to guide the clinical practice.Methods: 60 cases with malignant tumors in this research were divided into 3 groups according to the principle of randomization and blind method (i.e. Geshu Group, Zusanli Group, and Control Group), and each group contains 20 cases. Control Group was routinely applied drugs that can resist the side-effects of chemotherapy, while Geshu Group and Zusanli Group were added electrical acupuncture on Geshu or Zusanli of both sides on this foundation. Mainly observe the effect of using electrical acupuncture on Geshu and Zusanli to relieve the side effect of chemotherapy (including the occurred relative symptoms and clinical indexes). The observation cycle is from the beginning of chemotherapy to the end of the treatment, and the acupuncture time is supposed to be one week.Result:1. The influence of acupuncture on the hematological system after chemotherapy:(1) Acupuncture can inhibit the decrease of white blood cell (WBC) and neutrophil (NEU). Comparing pre-chemotherapy and post-chemotherapy of these three groups themselves, the differences have significant meaning (P<0.05). Comparing with pre-chemotherapy, the values of WBC and NEU of Geshu Group and Control Group were apparently decreased after chemotherapy, while those of Zusanli Group were increased after treatment, which indicated that electrical acupuncture on Zusanli can raise the degree of WBC and NEU. The comparison of WBC and NEU after chemotherapy among the three groups, compare Zusanli Group and Geshu Group with Control Group respectively, there were remarkable differences both among the three groups (P<0.05). And the difference between Zusanli Group and Geshu Group also has significant meaning (P<0. 05). So we can drew a conclusion that Geshu and Zusanli can increase the WBC and the NEU in the blood by electrical acupuncture, and the effect of Zusanli is better than Geshu.(2) Electric acupuncture on Zusanli can inhibit the decrease of platelet(PLT). In the comparison between before chemotherapy and after chemotherapy, the PLT of Group Zusanli did not change much than before chemotherapy (P>0.05) ,but the PLT of Group Geshu and the Control Group were much less than before chemotherapy (P<0.05) . in the comparison among three group after chemotherapy, the difference has not significant meaning ( P>0.05 ). Thus it can be seen that acupuncturing Zusanli can improve the blood platelet while the effect of acupuncturing Geshu is not obvious.(3)Acupuncture has not obvious effect on the hemoglobin(HGB). In the comparison of three groups themselves between before chemotherapy and after chemotherapy, the value of HGB has not obvious change (P>0.05) , for the comparison among three groups, the difference has not significant meaning(P>0.05). That means HGB do not change obviously after chemotherapy and electric acupuncture on Geshu and Zusanli have not obvious effect on HGB.2. Acupuncture has not obvious effects on Liver function and renal function after chemotherapy. In the comparison of three groups themselves between before chemotherapy and after chemotherapy, alkali phosphatase (ALP) , aspartate aminotransferase (AST) , glutamate-pyruvate transaminase (ALT), bilirubin total (TB) , urea nitrogen (BUN) and creatinine (CREA) ect. Indexes of Liver function and renal function have no obvious change (P>0.05 ) . And the difference among three groups after chemotherapy also has notsignificant meaning (P>0.05) . That shows the Liver function and renal function do not change much after chemotherapy. And electric acupuncture on Zusanli and Geshu has not obvious effect on Liver function and renal function of the patients accepting chemotherapy.3. Acupuncture can inhibit the decrease of the Karaofsky physical force grade. In the comparison of three groups themselves between before chemotherapy and after chemotherapy, the Karnofsky physical force grade decreases obviously (P<0.05 ) . For the comparison among three groups, compare Zusanli Group and Geshu Group with Control Group respectively, the difference has significant meaning (P<0.05) . Comparing Zusanli group and Geshu group, the difference has not significant meaning (P>0.05) . That shows both electric acupuncture on Zusanli and on Geshu can inhibit the decrease of Karnofsky physical force grade, but the difference between them has not significant meaning.4 The effect of acupuncture on the symptom of alimentary system.(1) Acupuncture on Zusanli has advantages for stopping vomit. The rate of vomiting of the three groups is, for Geshu group 50%, Zusanli group 15% and Control Group 60%. Comparing pre-chemotherapy with post-chemotherapy, the occurrence ratio of vomiting after chemotherapy was obviously increased (P<0. 05) . The rate of vomiting of Zusanli Group was much less than Geshu Group and Control Group after chemotherapy (P<0.05) . But comparing the group of electric acupuncture on Geshu and Control Group, the difference has not significant meaning (P>0.05) . That shows electric acupuncture on Zusanli can obviously inhibit the vomit after chemotherapy, but the effect of electric acupuncture on Geshu is not obvious.(2) Acupuncture on Geshu can stop hiccup efficiently. The rate of hiccup of the three groups after chemotherapy is, for Geshu group 10%, Zusanli group 25% and Control Group 50%. Comparing Zusanli group and Geshu group with Control Group respectively, the difference has significant meaning (P<0.05) . Comparing Zusanli group and Geshu group, the difference also has significant meaning (P<0.05) . Both of electric acupuncture on Zusanli and on Geshu can inhibit hiccup and the effect of Geshu is better than Zusanli in stopping hiccup.(3) Acupuncture on Zusanli can stop poor appetite efficiently .The rate of poor appetite of the three groups after chemotherapy is, for Geshu group 70%, Zusanli group 50% and Control Group 80%. Comparing Geshu group and Control Group with Zusanli group respectively, the difference has significant meaning (P<0.05) . Comparing Geshu group and Control Group, the difference has not significant meaning (P>0.05) .That means acupuncture on Zusanli can decrease the poor appetite after chemotherapy efficiently, but the effect of electric acupuncture on Geshu is not obvious.(4) Acupuncture has little effect on diarrhea, constipation, and dental ulcer, etc. Comparing with the condition before chemotherapy, the ratio of the occurrences of diarrhea, constipation, and dental ulcer after chemotherapy obviously increased (P<0.05), but there is no significant difference among these three groups (P>0.05). This indicates that electrical acupuncture on Geshu and Zusanli can not reduce the occurrence ratio of diarrhea, constipation, and dental ulcer. However, it was found that electrical acupuncture on Zusanli can relieve diarrhea in clinic.5. Acupuncture has little effect on the toxicity evaluation after chemotherapy. The... |
