Objective To investigate the change of pancreatic β-cell function in diabetic patients under the differential situations, including forms in diabetes mellitus(DM), age, disease duration and body mass index (BMI) in diabetic patients and associated factors by intravenous glucagon stimulation test (GST) and to explore the significance of clinical differential diagnosis and the application of GSTMethods 82 diabetic patients, including 14 type 1 diabetic patients(T1DM-group), 38 type 2 diabetic patients(T2DM group) and other 3 pairs of subgroups were studed. 68 T2DM patients were divided into age, disease duration and BMI groups according to differential age, disease duration and BMI. Age group was divided into ≤50 year-old(n=14)and >50 year-old(n=13)subgroups,which have similar disease duration and BMI.Desease duration group was divided into ≤lOyeres(n=18) and >10yeres(n=17)subgroups,which have similar age and BMI. BMI group was divided into <25kg/m2 and ≥25kg/m2 subgroups, which have similar age and disease duration. Healthy control group has 10 persons. Serum C-peptide (Op) levels of fasting and 6 minutes after glucagon injection were measured by radioimmunoassay, meanwhile the plasma glucose levels measured in all the groups.Results Fasting C-p levels were lower in T1DM group than those in healthy control group(p<0.005). But there were no significant differences in fasting C-p levels between T1DM and T2DM group and between T2DM and healthy control group(p>0.05). 6 min C-p levels after glucagons injection in T1DM group were lower than those in healthy control group(p<0.001) and T2DM group(p<0.01). There were no significant differences in 6 min C-p levels between T2DM group and healthy control group(p>0.05).Increasing percentage of C-p in T1DM group was lower than that in healthy control group(p<0.05), lower than that in T2DMgroup , but no significant differences(p>0.05). There were same differences between T2DM group and healthy control group(p>0.05).6 min C-p levels were lower in ≤50-year-old group than those in healthy control group(p<0.02). There were no significant differences in fasting C-p levels , 6 min C-p levels and increasing percentage C-p between ≤50-year-old and >50-year-old group(P>0.05).6 min C-p levels after glucagon injection in disease duration >10 years group were lower than those in ≤10 years group(p<0.001). Fasting C-p levels and 6 min C-p levels in ≥25kg/m2 group were higher than those in <25kg/m2 group(p<0. 05).Conclusion forms in DM, disease duration and BMI in diabetic patients are associated factors on pancreatic β-cell function in diabetic patients. Intravenous glucagon stimulation test has important clinical application to distinguish the differences of pancreatic β-cell function in diabetic patients and explore its associated factors. |
