CT And MRI Diagnosis And Differential Diagnosis Of Complex Adnexal Masses

Objective: 1, To determine the value of spiral CT and MR in the diagnosis and differential diagnosis of pelvic adnexal mass;2, To evaluate the diagnostic value of cystic follicles in qualitative diagnosis of adnexal masses.Method: CT findings in 92 patients and MRI imaging in 35 cases with pathologically-proved adnexal masses were retrospectively analyzed.Results: 1, The overall accuracy of CT was 75.5%, while the sensitivity was 88.6%. About half of inflammatory masses, tuberculosis, endometriosis and uterus myoma with cystic degeneration were falsely diagnosed for tumor. In addition, a co-existent was present in the 14% cases. All benign cystic lesions showed a smooth border and were often multilocular with higher tensility. Inflammatory masses were markly enhanced compared with benign cystic lesion.2, In 42 malignant ovarian tumor, 95% presented as a cystic mass with solid components or only a solid mass which were enhanced moderately. The overall accuracy of CT diagnosis of malignant masses was 89%. Enhanced peritoneal nodular, thickened omentum and abundant ascites were usually suggested for stage III tumor but the distribution of ascites was no variation in benign and malignant lesions.7% cases had both benign and malignant tumors.3, The sensitivity, specificity, and the overall accuracy of MR in the diagnosis of adnexal masses were 90%, 81% and 87% for each. Cystic follicles were significantly different in benign and malignant ovarian tumors.Conclusion: 1, Benign ovarian masses usually have some characteristic CT/MR signs. The diagnosis of diseases such as adnexal inflammatory masses(including tuberculosis), endometriosis, uterus momma with cystic formation should be combined with clinical information;2, Spiral CT and MR scan have high accuracy for staging malignant ovarian tumor, especially for stage â…¢ cases;3, Cystic follicle which is best showed on T2WI is helpful for qualitative diagnosis of adnexal masses.
