Objective: This experiment is designed to study on the porcine orthotopic liver transplantation without veno-venous bypass model, in order to grasp the key of operation, to inquire the changes of physiopathology in the recipient during different period in this operation and to obtain the principal managements against those changes, then probed into the feasibility and the foreground of this model.Materials and methods: Meishan pigs were selected and divided into as donor or recipient at random, established orthotopic liver transplantation without veno-venous bypass model performed 10 operations. Observed the details of operations and the postoperative last time with natural condition, immediately opened the abdominal cavity to inspect after die. During different period in the operation, including four points as following: Pre-anhepatic, Anhepatic(10min), Neohepatic(10min) and Before-Terminati-on, the blood samples of recipient were getting to examine the changes in blood gas analysis, biochemistry and blood coagulate function by the common jugular artery and the internal jugular vein, simultaneous recoded the changes of hemodynamics as HR, MAP, CVP.Results: Ten cases porcine orthotopic liver transplantation operationhad been performed without veno-venou:; bypass, the mean time of recipient s operation was 216 22min, the anhepatic stage was 42 13min. The abnormity of surgery been found by checked the postoperative natural death, bile leak, portal vein torsion and hepatic artery embolism was each one. There were transient changes in hemcdynamic, blood gas analysis, biochemistry and blood coagulate function dramatically during the anhepatic stage and the neohepatic stage, but by active treatment the internal environment were recovered nearly the pre-snhepatic stage rapidly before-termination of operation, except the blood coagulate function as APTT andFg.Conclusion: (1) It is important keys to establish the Porcine Orthotopic Liver Transplantation Without Veno-venous Etypass Model that the exquisite surgical and the effective measures of prevention or rectifications for the changes of internal environment. (2) The Porcine Orthotopic Liver Transplantation Without Veno-venous Bypass Model is a fine simulation of clinic liver transplantation, not only in operation skills but also in character changes of physiopathology and in treatments during the operation. So it can not be substitute in the training of specialists with clinic liver transplantation. (3) The Porcine Orthotopic Liver Transplamtation Without Veno-venous Bypass Model is utility and credibility, it is very suitable to use for the research in the field related the basement of liver transplantation and has ainestimable worthiness at future.
