The Analysis Of 47 Cases Of Ovarian Clear Cell Carcinoma

Ovarian clear cell carcinoma is a kind of epithelial ovarian tumor. Until now, the clinical characters and biological characters are still unclear. Recently lots of research considered it as a poor prognosis, low suvival rate, easy to relapse, fast metastasis and resistant to the rutine chemotherapy drugs. Ovarian clear cell carcinoma has more malignant biological characters than other epithelial ovarian tumor. There are little efficient treatment methods. This paper retrospectly reviewed 47 cases of ovarian clear cell carcinoma and analyzed the clinical characters, prognosis and other related factors.47 cases were divided into 4 groups by FIGO stages(Ⅰa+Ⅰb, Ⅰc,Ⅱ,Ⅲ,Ⅳ).We discussed their suvival rates seperately. We oberserved these cases till Dec.1st 2003 and consided the case as relapse one if there any conditions happened as followed. There is no tumor found for 6 months after completed operation and chemotherapy, and then there is a tumor again found in abdomen or the blood markers increasing again. All the datas were examined by SPSS 11.5 .We used χ2 test to do stastical counting and used Kaplan-Meier to count the suvival rates .47 cases of ovarian clear cell carcinoma were all done the tumor reduced operation including satisfied operation 31 cases (65.96%) and unsatisfied operation 16 cases (34.04%) .After the operation, the cases all continued to use chemotherapy. The median of chemotherapy duration is 6 (1-16) .There are 12 cases relapsed and 16 cases dead (34.04%) including 2 cases ofⅠa+Ⅰb, 3 cases of Ⅰc ,2 cases of Ⅱ ,1 case of Ⅲand 8 cases of Ⅳ.There are 12 cases of ovarian clear cell carcinoma combined with endometriosis and 35 cases of simply ovarian clear cell carcinoma. There is no difference in suvival rate between these two groups(P<0.05).Acoording FIGO stage,1-year suvival rate is 80.60%,3-year is 63.91%,5-year is 57.52%,5-year suvival rate has no significant difference. The cases which had been done satisfied tumor reduced operation have no significant difference with the unsatisfied group. These cases are all done chemotherapy after operation. The chemotherapy duration is 21 days. When the chemotherapy durations are above 6, the suvival rate has a increasing obviously. There are 31 cases of durations≥6, its 1-year suvival rate is 96.67%,3-year is 71.17%,5-year is 61.01%.And there are 16 cases of durations＜6, its 1-year suvival rate is 50.00%,3-year is 50.00%,5-year is 50.00%.These datas showed difference in 1-year survival rate and no difference in other suvival rates.Above all, ovarian clear cell carcinoma is a kind of ovarian epithelial tumor which has no significant clincal characters and always detected when physical eximination. It often simply occurrs one side ovary, and related with the age of women. These characters showed its development maybe relates with estrogen. The prognosis of ovarian clear cell carcinoma maybe has relationship with the factors as followed:FIGO stage,combined with endometriosis or not and doing rutine chemotherapy after operation or not. If the case is a early stage or combines with endometriosis or has been done rutine chemotherapy after operation, ovarian clear cell carcinoma has a better prognosis relatively. There isn't any difference no matter whether the satisfied tumor reduced operation.
